The present disclosure belongs to the field of biological medicines, and relates to use of an anti-CD47 antibody and a second therapeutic agent for combating a hematologic tumor, and a pharmaceutical combination.
CD47, also known as integrin-associated protein, is a transmembrane glycoprotein widely expressed among a variety of species and tissues. It is a member of the immunoglobulin superfamily. Inhibitory receptor signal-regulatory protein α (SIRPα) is one of the ligands of CD47, which binds to the NH2-terminal IgV-like domain of SIRPα. SIRPα is expressed primarily in myeloid-derived cells, including macrophages, granulocytes, DC cells, mast cells, and their precursors, including hematopoietic stem cells. CD47 expression and/or activity has been implicated in a number of diseases and disorders. Several different studies have shown that almost all tumor cells and tumor tissues highly express CD47. CD47 highly expressed on the tumor cell surface binds to SIRPα on the macrophage surface, sending a “don't eat me” signal, so that the macrophages in the tumor tissue infiltration area get along with the tumor cells and even facilitate the expansion and growth of the tumor cells by facilitating the proliferation of blood vessels in tumors and suppressing effector T cells. Drugs targeting the CD47-SIRPα axis, such as anti-CD47 antibodies, are currently in clinical trials.
At present, there is still an unmet clinical need for the treatment of cancer, particularly a hematologic tumor.
The present disclosure provides use of an anti-CD47 antibody and a second therapeutic agent for preparing a medicament for treating a hematologic tumor in a subject.
The present disclosure provides a method for treating a hematologic tumor in a subject, comprising administering to the subject a therapeutically effective amount of an anti-CD47 antibody and a second therapeutic agent.
The present disclosure also provides a pharmaceutical combination comprising an anti-CD47 antibody and a second therapeutic agent.
The present disclosure also provides a pharmaceutical combination for treating a hematologic tumor, comprising an anti-CD47 antibody and a second therapeutic agent.
In some embodiments, the second therapeutic agent described above is selected from the group consisting of a hypomethylating agent, an anti-CD20 antibody, an immunomodulator, and a proteasome inhibitor.
In some embodiments, the anti-CD47 antibody is the anti-CD47 antibody provided herein.
The present disclosure also provides a kit comprising the pharmaceutical combination.
The present disclosure provides use of the anti-CD47 antibody herein for preparing a medicament for treating myeloma in a subject. Also provided is a method for treating myeloma in a subject, comprising administering to the subject a therapeutically effective amount of the anti-CD47 antibody herein.
The term “antibody” is used in its broadest sense and encompasses a variety of antibody structures, including natural and artificial antibodies of various structures, including, but not limited to, monoclonal antibodies, monospecific antibodies, multispecific antibodies (e.g., bispecific antibodies, trispecific antibodies, etc.), single-chain antibodies, antibody fragments, and the like, so long as they exhibit the desired antigen-binding activity. The antibody may be an intact antibody. An antibody fragment refers to a molecule other than an intact antibody, which comprises a portion of the intact antibody that binds to the antigen to which the intact antibody binds. Examples of antibody fragments include, but are not limited to, Fv, Fab, Fab′, Fab′-SH, F(ab′)2, single-chain antibody molecules (e.g., scFv), and single-domain antibodies.
The term “isotype” refers to the class of antibodies encoded by the heavy chain constant region gene.
The term “chimeric antibody” refers to an antibody having at least a portion of a heavy chain variable region and a portion of a light chain variable region derived from one species, and at least a portion of a constant region derived from another species. For example, in some embodiments, the chimeric antibody may comprise a murine variable region and a human constant region.
The term “humanized antibody” refers to an antibody comprising complementarity determining regions (CDRs) derived from a non-human antibody, and framework and constant regions derived from a human antibody. For example, the humanized antibody that binds to CD47 provided herein may comprise CDRs derived from one or more murine antibodies and human framework and constant regions.
The term “variable domain” or “variable region” refers to a domain of an antibody that is involved in the binding of the antibody to an antigen. For example, a natural four-chain antibody (e.g., derived from human, murine, and the like) has a heavy chain variable region (VH) and a light chain variable region (VL), and a heavy-chain antibody derived from an animal such as camelid or shark has a single variable domain. In most cases, each variable domain of a natural antibody consists substantially of four “framework regions” and three “complementarity determining regions”. The four framework regions are referred to as framework region 1 (FR1), framework region 2 (FR2), framework region 3 (FR3), and framework region 4 (FR4), respectively. The framework regions are separated by three complementarity determining regions (CDRs) referred to in the art and hereinafter as complementarity determining region 1 (CDR1), complementarity determining region 2 (CDR2), and complementarity determining region 3 (CDR3), respectively. Thus, the general structure of a variable domain can be represented as follows: FR1-CDR1-FR2-CDR2-FR3-CDR3-FR4. In one specific example, the general structure of a heavy chain variable region can be represented as follows: FR1-HCDR1-FR2-HCDR2-FR3-HCDR3-FR4. In one specific example, the general structure of a light chain variable region can be represented as follows: FR1-LCDR1-FR2-LCDR2-FR3-LCDR3-FR4. Variable domains impart specificity for an antigen to an antibody by virtue of having an antigen-binding site. “CDR” (complementarity determining region), also referred to “hypervariable region (HVR)”, typically refers to each region of antibody variable regions which are highly variable in sequence and/or form structurally defined loops. A natural four-chain antibody typically comprises six CDRs, among which three are in the heavy chain variable region, i.e., heavy chain CDR1 (HCDR1), heavy chain CDR2 (HCDR2) and heavy chain CDR3 (HCDR3), and three are in the light chain variable region, i.e., light chain CDR1 (LCDR1), light chain CDR2 (LCDR2) and light chain CDR3 (LCDR3). A heavy-chain antibody or a single variable domain typically has three CDRs (CDR1, CDR2, and CDR3).
There are currently many ways to define CDRs. The Kabat scheme defines CDRs based on the sequence variability and is the most commonly used (Elvin A. Kabat, et al., Sequences of Proteins of Immunological Interest, 5th Edition, Public Health Service, National Institutes of Health, Bethesda, Md. (1991)), while the Chothia scheme defines CDRs based on the positions of structural loops (Cyrus Chothia, et al., Canonical Structures for the Hypervariable Regions of Immunoglobulins, J Mol. Biol. 196:901-917(1987)). The AbM scheme, a compromise between the Kabat scheme and the Chothia scheme, is used by the Oxford Molecular's AbM antibody modeling software. “Contact” defines CDRs based on the analysis of the crystal structure of available complexes. However, it should be noted that boundaries of the CDRs of variable regions of the same antibody based on different methods and definitions may differ, that is, CDR sequences of the variable regions of the same antibody defined by different methods may differ. Thus, when reference is made to an antibody defined with a particular CDR sequence defined herein, the scope of the antibody also encompasses antibodies defined by CDR sequences that are converted to other arbitrary definitions (e.g., Chothia, AbM definitions, etc.).
The term “framework region” (FR) refers to amino acid residues of variable domains other than the CDR residues defined herein.
The term “isolated” means that a target compound (e.g., an antibody or nucleic acid) has been isolated from its natural environment.
As used herein, the term “EC50” refers to the effective concentration, 50% of the maximal response of an antibody.
As used herein, the term “IC50” refers to the inhibitory concentration, 50% of the maximal response of an antibody.
Both EC50 and IC50 can be measured by ELISA or FACS assay or any other method known in the art.
The term “KD” as used herein refers to the equilibrium dissociation constant, expressed in molar concentration (M).
The KD value of an antibody can be determined using methods well known in the art. One preferred method for determining the KD of an antibody is performed by using surface plasmon resonance, more preferably a biosensor system, such as a Biacore system.
As used herein, the term “subject” includes any human or non-human animal. The term “non-human animal” includes all vertebrates, e.g., mammals and non-mammals, such as non-human primates, sheep, dogs, cats, horses, cows, chickens, amphibians, and reptiles. Preferably, the subject according to the present disclosure is a human. The terms “patient” and “subject” are used interchangeably unless otherwise indicated.
The terms “hematologic malignancy”, “hematologic tumor”, and “hematologic cancer” are used interchangeably herein in the broadest sense and refer to all stages and forms of cancer caused by cells of the hematopoietic system. The term “treating” or “treatment” refers to an attempt to alter the natural course of a disease in a treated individual, and may be a clinical intervention performed for prophylaxis or during the course of clinical pathology. Desired therapeutic effects include, but are not limited to, preventing the occurrence or recurrence of diseases, relieving symptoms, reducing any direct or indirect pathological outcomes of diseases, preventing metastasis, slowing disease progression rates, improving or alleviating disease conditions, and regressing or improving prognosis.
As used herein, the term “therapeutically effective amount” refers to the amount of a compound, a composition, or a pharmaceutical combination necessary to provide a therapeutic benefit to a subject.
The term “non-fixed combination” refers to simultaneous, parallel, or sequential and temporally unlimited administration of two or more active components as independent entities (for example, pharmaceutical compositions and formulations) to a subject, wherein the active ingredients administered to the subject reach therapeutically effective amounts. An example, which can be listed, of the non-fixed combination is a cocktail therapy, for example, 3 or more active components are administered. In a non-fixed combination, each of the active components can be packaged, sold or administered as a fully independent pharmaceutical composition.
The term “fixed dose” refers to a dose administered to a patient without considering the body weight or the body surface area (BSA) of the patient. Thus, the fixed dose is specified as an absolute amount of an agent (e.g., anti-CD47 antibody) rather than, for example, an mg/kg dose. For example, a human of 80 kg and a human of 100 kg will receive the same dose of antibody (e.g., 800 mg of the anti-CD47 antibody). In a manner different from the fixed dose, it is also possible to perform administration in a dose calculated in accordance with the body weight or body surface area of the patient, for example, in a dose of 0.5 mg/kg to 70 mg/kg in accordance with the body weight of the patient.
The term “single dose” refers to the smallest unit of packaging containing a certain quantity of pharmaceutical product; for example, in a box of seven capsules, each capsule is a single dose; or a vial of injection is a single dose.
As used herein, the terms “single dose” and “unit dose” have the same meaning and are used interchangeably.
“Pharmaceutical composition” refers to a composition comprising an active ingredient and a pharmaceutically acceptable carrier.
The “percent identity (%)” of an amino acid sequence refers to the percentage of amino acid residues in a sequence to be aligned that are identical to those of a specific amino acid sequence as set forth herein when the sequence to be aligned is aligned with the specific amino acid sequence as set forth herein, with gaps introduced, if necessary, to achieve the maximum percent sequence identity and without any conservative replacements as part of the sequence identity. Alignment of amino acid sequences for identity can be performed in a variety of ways within the skill in the art, such as BLAST, BLAST-2, ALIGN, or Megalign (DNASTAR) software. Those skilled in the art can determine suitable parameters for aligning sequences, including any algorithms required to obtain maximum alignment for the full length of sequences being compared.
The terms “Xn” and “Xaa” are equivalent and refer to an unspecified amino acid whose scope is specified by the subsequent definitions in the relevant description.
The term “include”, “contain”, or “comprise” and variants thereof should be understood as “including, but not limited to”, which means that other elements, components and steps that are not specified are encompassed in addition to the elements, components and steps that are listed.

The anti-CD47 antibody provided herein is suitable for treatment of hematologic tumors, alone or in combination with other therapeutic agents. In addition, the combination described above may also have one or several of the following advantages:
(1) In some experiments, it was found that the combination of the anti-CD47 antibody and other therapeutic agents (e.g., anti-CD20 antibodies, hypomethylating agents, etc.) can enhance the anti-tumor effect of a single agent, e.g., killing tumor cells, inhibiting tumor growth, or/and eliminating tumors, etc. In particular, for example, in some hematologic tumors such as lymphomas, leukemias, myelodysplastic syndromes, multiple myelomas, etc., it has good efficacy or/and safety.
(2) The combination of the anti-CD47 antibody provided herein and other therapeutic agents can provide a treatment that is well tolerated in a subject with good safety, e.g., with fewer adverse reactions or/and complications.
(3) The combination of the anti-CD47 antibody provided herein and other therapeutic agents can allow for administration in smaller amounts (e.g., administration amounts or/and total administration amounts) and benefit from the administration as compared to a single agent or some other known agents or combination of agents.
The present disclosure provides use of an anti-CD47 antibody and a second therapeutic agent for preparing a medicament for treating a hematologic tumor in a subject.

The present disclosure also provides a pharmaceutical combination comprising the anti-CD47 antibody described above and the second therapeutic agent described above.
In some embodiments, the pharmaceutical combination is a non-fixed combination. The anti-CD47 antibody and the second therapeutic agent in the non-fixed combination are each in the form of a pharmaceutical composition.
In some embodiments, the pharmaceutical combination is for use in treating the hematologic tumor described above herein. In some embodiments, provided is a pharmaceutical combination for treating the hematologic tumor described above herein, comprising the anti-CD47 antibody described above and the second therapeutic agent described above.
In some embodiments, the present disclosure provides a kit comprising the pharmaceutical combination. The kit may include instructions directing administration of the pharmaceutical combination to a subject with cancer. The kit may also include other materials as may be required from a commercial and user standpoint, for example, other buffers, diluents, needles, syringes, and the like.
In some embodiments, the second therapeutic agent described herein is a hypomethylating agent. Thus, in some embodiments, provided is use of an anti-CD47 antibody and a hypomethylating agent for preparing a medicament for treating a hematologic tumor in a subject. In some embodiments, provided is a method for treating a hematologic tumor in a subject, comprising administering to the subject a therapeutically effective amount of an anti-CD47 antibody and a hypomethylating agent.
In some embodiments, the hypomethylating agent is azacitadine (5-azacytidine), decitabine (5-azadeoxycytidine), guadecitabine (SGI-110), zebularine, or procaine. In some specific embodiments, the hypomethylating agent is azacitadine.
Azacitadine is a cytosine analog, which is an inhibitor of nucleoside metabolism, with the chemical name of 4-amino-1-β-D-ribofuranosyl-1,3,5-triazin-2(1H)-one and the molecular formula of C8H12N4O5. One example of a marketed drug is the drug VIDAZA®, with azacitadine as the active ingredient. Another example of a marketed drug is the generic drug , with azacitadine as the active ingredient.
In some embodiments, the anti-CD47 antibody is selected from the group consisting of those described in the section of “Anti-CD47 Antibody”. In some embodiments, the hematologic tumor is leukemia, lymphoma, myelodysplastic syndrome, or myeloma. In some embodiments, the leukemia is acute leukemia (AL) or chronic leukemia (CL). In some embodiments, the leukemia is acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), chronic myeloid leukemia (CML), chronic lymphocytic leukemia (CLL), or rare types of leukemia.
In some embodiments, the hematologic tumor is acute myeloid leukemia.
In some embodiments, the acute myeloid leukemia is recurrent or/and refractory AML.
In some embodiments, the acute myeloid leukemia includes, but is not limited to, various subtypes. Generally, the acute myeloid leukemia can be divided into different subtypes according to different classification criteria or systems. For example, in some embodiments, the acute myeloid leukemia is a subtype under the FAB criteria or under the WHO criteria. In some embodiments, the MDS is MDS with single lineage dysplasia (MDS-SLD), MDS with multiple lineage dysplasia (MDS-MLD), MDS with ring sideroblasts (MDS-RS), MDS with excess blasts (MDS-EB), or MDS, unclassifiable (MDS-U).
In some embodiments, the MDS is low-risk, intermediate-risk, or high-risk MDS.
In some embodiments, the MDS has genetic abnormalities, including those selected from the group consisting of, for example, a chromosomal abnormality of +8, −7/del(7q), del(20q), −5/del(5q), or —Y, and for example, a gene mutation of TP53 mutation, TET2 mutation, DNMT3A mutation, IDH1/2 mutation, EZH2 mutation, ASXL1 mutation, SRSF2 mutation, RUNX1 mutation, U2AF1 mutation, SETBP1 mutation, or SF3B1 mutation, and the like.
The present disclosure also provides a pharmaceutical combination comprising the anti-CD47 antibody described above and the hypomethylating agent described above.

In some embodiments, when administered to a subject with cancer in a therapeutic dose, the anti-CD47 antibody may also be administered in a fixed dose. In some embodiments, the anti-CD47 antibody is administered in a fixed dose of 60 mg to 3000 mg, e.g., 60 mg to 2000 mg, 600 mg to 1800 mg, or 1000 mg to 1800 mg. Suitable fixed doses include, for example, 60 mg, 100 mg, 180 mg, 360 mg, 500 mg, 600 mg, 700 mg, 800 mg, 900 mg, 1000 mg, 1100 mg, 1200 mg, 1300 mg, 1400 mg, 1500 mg, 1600 mg, 1700 mg, 1800 mg, 1900 mg, 2000 mg, 2500 mg, or 3000 mg.
In some embodiments, the anti-CD47 antibody is administered in one, two, or more therapeutic doses every 3-21 days, e.g., every 3-14 days or 7-14 days. Suitable administration frequencies are, for example, administration of a therapeutic dose of the anti-CD47 antibody twice a week, once a week (q1w), once every 2 weeks (q2w), or once every 3 weeks (q3w).
In some embodiments, the anti-CD47 antibody can typically be administered at a therapeutic dose between 1 and 6 times (e.g., 1, 2, 3, 4, 5, or 6 times) to treat cancer (such as liver cancer), and can be administered at a therapeutic dose of 7, 8, 9, 10, 11, 12, 13, 14, or more times.
In some embodiments, a therapeutic dose of the anti-CD47 antibody is administered to a subject with cancer in one treatment cycle of every 2-6 weeks, e.g., one treatment cycle of every 2 weeks (14 days), every 3 weeks (21 days), every 4 weeks (28 days), every 5 weeks (35 days), or every 6 weeks (42 days).
In some embodiments, it is permissible to administer the anti-CD47 antibody in a dose escalation manner to achieve the therapeutic dose.
In some embodiments, it is permissible to administer a sub-therapeutic dose of the anti-CD47 antibody that is less than the therapeutic dose prior to administration of the therapeutic dose of the anti-CD47 antibody.
The anti-CD47 antibody can be administered by a variety of routes, in one embodiment, intravenously.
Regimens such as administration interval, administration dose, and route of administration for the second therapeutic agent may be made in accordance with the properties of each therapeutic agent itself. For example, rituximab may be administered intravenously, azacitadine may be administered by subcutaneous injection, lenalidomide may be administered orally, and bortezomib may be administered intravenously.
In some embodiments, the anti-CD47 antibody may be administered simultaneously, sequentially, or at an interval when used in combination with a second therapeutic agent.
Some exemplary anti-CD47 antibodies suitable for use in any of the uses, methods, or pharmaceutical combinations described above are provided below.
In certain embodiments, in addition to binding to human CD47 or/and cynomolgus monkey CD47, the anti-CD47 antibody provided has a number of other desired characteristics for a targeted therapy. Specifically, these desired characteristics may be, for example, at least one of the following: binding to CD47 with a KD value of 1.53E-08 or less; blocking binding of CD47 to SIRPα; promoting macrophage-mediated phagocytosis of CD47-expressing cells; not significantly inducing apoptosis of CD4+ T cells; not causing substantial hematocytopenia, anemia or red cell agglutination; and the antigen-binding polypeptide or the antigen-binding portion having a melting temperature T of ≥62° C. and an aggregation temperature Tagg of ≥61° C. In some embodiments, the anti-CD47 antibody has a better therapeutic effect; in some embodiments, the anti-CD47 antibody has reduced side effects.

The anti-CD47 antibody can be prepared using methods well known in the art. For example, a host cell comprising a nucleic acid encoding the anti-CD47 antibody is cultured under conditions suitable for expression of the anti-CD47 antibody, and the anti-CD47 antibody is isolated from the host cell or host cell culture medium. To produce an anti-CD47 antibody, a nucleic acid encoding the antibody is isolated and inserted into one or more vectors for further cloning or/and expression in a host cell. The nucleic acid can be obtained using various methods known in the art, such as gene cloning, gene splicing and chemical synthesis. The isolation can be performed through a step such as centrifugation or affinity chromatography.

Immunization of Mice with CD47 Protein and Acquisition of Antibody Variable Region Sequences:
To produce antibodies against CD47, Balb/c mice were immunized subcutaneously with a recombinant hCD47-His Tag protein (ACRObiosystems, Cat. No. CD7-H5227) (50 μg/Balb/c mice) or hCD47-mFc (ACRObiosystems, Cat. No. CD7-H52A5) emulsified in an equal volume of Freund's complete adjuvant (first immunization) or Freund's incomplete adjuvant (booster immunization) every 2 weeks for three times over a period of 6 weeks. In the fourth immunization, each mouse was injected intramuscularly with 20 μg of hCD47-mFc (ACRObiosystems, Cat. No. CD7-H52A5) protein (in aqueous adjuvant). Three days prior to cell fusion, mice were injected intravenously with an antigen without an adjuvant to boost the immunization. Spleen cells (1×108) from the immunized mice were fused with SP2/0 myeloma cells (1.5×107) using polyethylene glycol 1500 (Roche, Cat. No. 10783641001).
The cloned hybridoma cells were preserved and used for extraction of cDNA to obtain antibody variable region sequences. The heavy chain variable region (VH) sequences and light chain variable region (VL) sequences of five murine monoclonal antibodies (2B2, 2H8, 3F10, 16E5 and 14A9) were obtained from the corresponding hybridoma clones (2B2, 2H8, 3F10, 16E5 and 14A9) by amplification and sequencing. The information on the sequences is shown as follows:
Through the chemical synthesis of genes of mouse antibody variable regions, the VL region of each mouse antibody was linked to the human κ chain constant region to construct a chimeric light chain, and the mouse VH region was linked to the IgG4 (with the EU number of S228P) constant region to construct a chimeric heavy chain. Expression plasmids for the chimeric light chain and the chimeric heavy chain were constructed by conventional genetic engineering means. CHO cells (a 200 mL system at 106 cells/mL) were transfected with 100 μg of each of the expression plasmids for the chimeric heavy chain and the chimeric light chain, and then cultured for 6 days. Then the chimeric antibody in the supernatant was purified using a protein A column.
Five chimeric antibodies were obtained by construction and expression. CHO cells (a 200 mL system at 106 cells/mL) were transfected with 100 μg of each of the expression plasmids for the heavy chain and light chain of the humanized antibodies, and then cultured in an ExpiCHO medium (Gibco, Cat. No. A29100-01) in an incubator at 37° C. with 5% CO2 for 6 days. After centrifugation, a supernatant was obtained, and the humanized antibodies in the supernatant were purified using a protein A column.
The equilibrium dissociation constants KD (in unit M) were obtained by measuring the binding kinetics of the human CD47 protein (human CD47 protein, His Tag; ACRObiosystems, Cat. No. CD7-H5227) and cynomolgus monkey CD47 protein (cynomolgus/rhesus macaque CD47 protein, His Tag; ACRObiosystems, Cat. No. CD7-C52H1) to anti-CD47 antibodies (chimeric or humanized) using Biacore. The Biacore analysis was performed at 25° C. and data were recorded at a rate of 1 Hz.
Polyclonal rabbit anti-mouse IgG (GE, BR-1008-38) was diluted with 10 mM sodium acetate at pH 5.0 and immobilized onto the reference flowcell and experimental flowcell of a CM5 biosensor chip using an amine coupling kit (GE, BR10050), and about 15,000 RM was reached. At the beginning of each cycle, the diluted test antibody (1.5 μg/mL) was injected onto the experimental flowcell and maintained for 1 min to be captured. These data showed that the anti-CD47 antibodies (chimeric and humanized) bound to CD47 as measured by Biacore.
The ability of the anti-CD47 antibodies to promote the phagocytosis of tumor cells by macrophages having SIRPα on the cell surface was determined based on flow cytometry.
Monocytes were isolated from human peripheral blood mononuclear cells (PBMC, Sailybio, Cat. No. 190056) using a monocyte isolation kit (STEMCELL, Cat. No. 19058), plated into a 24-well plate (Greiner bio-one, Cat. No. 662-160) at 5×10′ cells per well, and induced under 100 ng/mL stimulating factor M-CSF (R&D Systems, Cat. No. 216-MC-010) for 7 days to form macrophages. 1×106 target cells HL60 (Shanghai Institute of Biochemistry and Cell Biology, Chinese Academy of Sciences, Cat. No. TCHu23) were suspended in 5 αmol/L CFSE (BD Biosciences, Cat. No. 565082) fluorescent dye (diluted with a PBS buffer to the desired concentration), incubated in a water bath at 37° C. in the dark for 15 min, and washed with a PBS buffer to prepare CFSE-labeled HL60 cells, which were then incubated with each of 10 μg/mL and 1 μg/mL anti-CD47 antibodies (see Table 3 for chimenic antibodies, and see Table 4 for humanized antibodies) in an incubator at 37° C. with 5% CO2 for 1-2 h. The HL60 cells after the incubation were washed three times with a PBS buffer and added to the macrophages. The cells were incubated in an incubator at 37° C. with 5% CO2 for 2-4 h. After the cells were washed three times with a PBS buffer, a mixture of 1260 μL of PBS, 20 μL of anti-APC-CD14 (APC MouseAnti-Human CD14, BD Biosciences, Cat. No. 555399), and 20 μL of PE-CD11b (PE Mouse Anti-Human CD11b/1Mac-1, BD Biosciences, Cat. No. 555388) antibody was added to each well. The cells were incubated at 4° C. in the dark for 30 min, washed three times with a PBS buffer, and digested with trypsin (Gibco, Cat. No. 12604-013). The rate of phagocytosis (%) by the macrophages was determined using a flow cytometer (BD Biosciences, model: C6).
The effects of the anti-CD47 chimeric antibodies and anti-CD47 humanized antibodies on macrophage phagocytosis are shown in Tables 3 and 4. Among the antibodies, the chimeric antibodies Xi3F10, Xi16E5 and Xi14A9, and the anti-CD47 humanized antibodies had good promoting effects on phagocytosis, and the humanized antibodies hz14A9-2.3 and hz14A9-2.4 showed better performance.
The high-throughput protein stability analyzer Prometheus NT.48 is an instrument for acquiring various types of data such as protein molecular structure stability, aggregation stability, and colloid dispersion stability. The melting temperatures (Tm) and the aggregation temperatures (Tagg) of the anti-CD47 chimeric antibodies and the anti-CD47 humanized antibodies were determined using this instrument. The results are shown in Tables 5 and 6. The results showed that the anti-CD47 antibodies had good thermostability.
A hematologic tumor mouse model was established using hematologic tumor Raji cells. The anti-CD47 antibodies were applied to the model for the evaluation of the pharmacodynamic activity.
A suspension of Raji cells in a PBS buffer was prepared at a concentration of 5×106 cells/mL, and was injected into female NOD/SCID mice via the tail vein at 0.1 mL/mouse. After 7 days, the mice were randomly divided into 4 groups of 10 each, which were group 1: IgG4 group; group 2: Hu5F9 group; group 3: hz3F10-6.1; and group 4: hz14A9-2.3. The administration was performed by intraperitoneal injection in doses of 10 mg/kg for 21 consecutive days. The survival time of the animals was observed.

Since CD47 is expressed on red cells and plays a role in clearing aged red cells, the toxic effects of the anti-CD47 antibodies in B-hCD47 mice were compared.
B-hCD47 mice were randomly divided into 3 groups: an Hu5F9 group of 3 mice, an hz14A9-2.3 group of 4 mice, and an hz3F10-6.1 group of 3 mice. The B-hCD47 mice were injected with an Hu5F9 antibody, an hz14A9-2.3 antibody, or an hz3F10-6.1 antibody via the tail vein in a single dose of 10 mg/kg, and changes in red blood cell count (RBC), hemoglobin (HGB) level, and body weight were determined 2, 4, 7, 10 and 14 days after administration.

The experimental animals were NOD-SCID mice (5-6 weeks old, female) purchased from Beijing Huafukang Biotechnology Co., Ltd., with the production license number of SCXK (Jing) 2019-0008 and the animal certification number of 110322211100910838. The housing environment was in a SPF grade. Human acute myelocytic leukemia MOLM-16 cells were purchased from DSMZ. MOLM-16 cells were cultured in an incubator at 37° C. with 5% CO2 in air. Subculturing was performed twice a week, and when the cells were in a logarithmic growth phase, the cells were collected, counted, and inoculated. 1×107 MOLM-16 cells were inoculated to each mouse subcutaneously, and when tumors reached the volume of 100-150 mm3, the mice were divided into groups and injected with drugs intravenously (IV) at the administration volume of 0.1 mL/10 g of body weight. The administration dosages and regimens are shown in Table 10.
The experimental index was to investigate the effects of the drugs on the tumor growth, and the specific index was T/C % or the tumor growth inhibition rate (TGI %, tumor growth inhibition).
Tumor diameters were measured twice a week with a vernier caliper and tumor volume (V) was calculated according to the following formula:

On D12 of the experiment, the tumor growth inhibition rate of hz14A9-2.3 (0.5/1.5 mg/kg, IV, DO, 3, 6, and 10) for the subcutaneous mouse xenograft tumor of human acute myeloblastic leukemia MOLM-16 was 11%; the tumor growth inhibition rates of azacitadine (1 mg/kg, IV, QD×5) and azacitadine (2.5 mg/kg, IV, QD×5) for the subcutaneous mouse xenograft tumor of MOLM-16 were 50% and 82%, respectively; the tumor growth inhibition rate of the combination of hz14A9-2.3 (0.5/1.5 mg/kg, IV, DO, 3, 6, and 10) and azacitadine (1 mg/kg, IV, QD×5) for the subcutaneous mouse xenograft tumor of MOLM-16 was increased to 58%; the tumor growth inhibition rate of the combination of hz14A9-2.3 (0.5/1.5 mg/kg, IV, DO, 3, 6, and 10) and azacitadine (2.5 mg/kg, IV, QD×5) for the subcutaneous mouse xenograft tumor of MOLM-16 was increased to 91% (D12).
When the experiment was prolonged to D17, the mean tumor volumes of the azacitadine (1 or 2.5 mg/kg, IV, QD×5) monotherapy groups were 3729.5+436.4 mm3 and 1920.7+375.6 mm3, respectively; the mean tumor volumes of the combinations of azacitadine and hz14A9-2.3 were 2447.0+502.4 mm3 and 922.4+297.5 mm3, respectively.
Among these groups, the combination group of hz14A9-2.3 and 2.5 mg/kg azacitadine showed a significant increase in the tumor growth inhibition rate and was significantly stronger than the monotherapy group (P<0.05, vs monotherapy) (

The housing environment was in a SPF grade. Human B-cell lymphoma Daudi cells were purchased from Cell Bank of the Chinese Academy of Sciences. Daudi cells were cultured in an incubator at 37° C. with 5% CO2 in air. Subculturing was performed twice a week, and when the cells were in a logarithmic growth phase, the cells were collected, counted, and inoculated. 1×107 Daudi cells were inoculated to each mouse subcutaneously, and when tumors reached the volume of about 100 mm3, the mice were divided into groups and injected with drugs intravenously (IV) once every 3 days (Q3D) for a total of 2 times at the injection volume of 0.1 mL/10 g of body weight. The administration dosages and regimens are shown in Table 11.

Primary objective: to evaluate the safety and tolerability of the anti-CD47 antibody alone (phase Ia) or in combination (phase Ib) in patients with advanced malignancies and determine the dose-limiting toxicity (DLT), maximum tolerated dose (MTD) (if any) or optimal biological dose (OBD), and recommended phase 2 dose (RP2D).
Secondary objectives: to evaluate the pharmacokinetic (PK) characteristics of the anti-CD47 antibody alone or in combination in patients with advanced malignancies; to evaluate the preliminary efficacy of the anti-CD47 antibody alone or in combination for advanced malignancies; to evaluate the immunogenicity of the anti-CD47 antibody; to evaluate the pharmacodynamic (PD) characteristics of the anti-CD47 antibody alone or in combination.
1. 18 years≤age≤75 years (based on the date of signing an informed consent); male or female without limitation; ECOG score: 0-1; expected survival >3 months.
2. For lymphomas, the following criteria must be at least met:
Histologically well-diagnosed lymphoma, including, but not limited to, classical Hodgkin's lymphoma and non-Hodgkin's lymphoma.
CD20-positive by an immunophenotypic analysis (only for the phase Ib of combination dose escalation).
Prior≥second-line adequate treatment and failure to achieve remission on the last adequate treatment or disease progression/recurrence after achieving remission (including recurrence after receiving autologous hematopoietic stem cell transplantation), CD20-positive disease also refractory to at least one regimen of anti-CD20 monoclonal antibody alone or in combination with chemotherapy (except for chronic lymphocytic leukemia/small lymphocytic lymphoma); or no standard of care; or unwillingness to accept existing approved therapies.
3. For leukemias, the following criteria must be at least met:
Acute myeloid leukemia (AML) or intermediate- to high-risk (IPSS-R) myelodysplastic syndrome (MDS) pathologically confirmed according to the 2016 World Health Organization (WHO) diagnostic criteria.
Recurrent/refractory disease, or intolerance to hypomethylating drugs or other drug treatments; or no other suitable treatment; or unwillingness to accept existing approved treatments.
4. For myelomas, the following criteria must be at least met:
Histologically confirmed multiple myeloma (MM).
Recurrent/refractory disease that has failed prior induction or consolidation or maintenance treatment, or intolerance to existing treatments; or no other suitable treatment; or unwillingness to accept existing approved treatments.
Experimental design and administration regimen:
This study was designed as an open-label, dose-escalation trial, including two phases of Ia of single dose escalation and Ib of combination dose escalation.
In the phase Ia of single dose escalation, eligible patients were sequentially assigned to 5 anti-CD47 antibody dose groups (60 mg, 180 mg, 600 mg, 1200 mg, and 1800 mg) in the order of enrollment. Each enrolled patient received the treatment with a target dose of the anti-CD47 antibody, tentatively by intravenous infusion once a week in one treatment cycle of every 4 weeks (28 days), for a maximum duration of treatment of no more than 2 years.
In the phase Ib of combination dose escalation, the MTD or OBD or RP2D dose or the administration regimen determined in the phase Ia of single dose escalation was selected, and the combination dose escalation was performed according to different indication cohorts. The patients with recurrent/refractory acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) were included in the cohort of the combination treatment of the anti-CD47 antibody and azacitadine, and the drug azacitadine for injection was a lyophilized powder injection for subcutaneous injection with the specification of 100 mg/vial, which was produced and supplied by Chia Tai Tianqing Pharmaceutical Group Co., Ltd. The patients with recurrent/refractory CD20-positive lymphoma were included in the cohort of the combination treatment of the anti-CD47 antibody and rituximab, and the drug rituximab for injection was a sterile injection for intravenous administration with the specification of 100 mg/10 mL, which was supplied by Nanjing Shunxin Pharmaceutical Co., Ltd. of Chia Tai Tianqing Pharmaceutical Group. The patients with recurrent/refractory multiple myeloma were included in the cohort of the combination of the anti-CD47 antibody and a proteasome inhibitor (bortezomib and the like) and/or an immunomodulator (lenalidomide and the like), and the drug bortezomib for injection was a lyophilized powder injection for intravenous administration (with the specification of 1 mg/vial and 3.5 mg/vial), which was produced and supplied by Chia Tai Tianqing Pharmaceutical Group Co., Ltd., and the drug lenalidomide capsule was an oral capsule with the specification of 25 mg, which was supplied by Nanjing Shunxin Pharmaceutical Co., Ltd. of Chia Tai Tianqing Pharmaceutical Group.
The inclusion/exclusion criteria, administration regimen, and study procedure of the phase Ib of combination dose escalation in the protocol would be revised in time according to the results and experience of the phase Ia of single dose escalation phase.
The evaluation of efficacy and safety was performed according to the standard.
The anti-CD47 antibody used in this study was an hz14A9-2.3 injection, which was a sterile injection for intravenous administration, and was produced and supplied by Nanjing Shunxin Pharmaceutical Co., Ltd. of Chia Tai Tianqing Pharmaceutical Group. This study also relates to the evaluation of the combination of the anti-CD47 antibody and other therapeutic agents, which is not described here.
