The present disclosure relates to the field of medical device technology, and more particularly, relates to a balloon catheter system with repair function.
Percutaneous intervention has become one of the commonly used treatment techniques worldwide. Balloon dilation and stent implantation are usually used for the treatment of intravascular stenosis lesions, however, both methods have their own shortcomings.
A possibility of restenosis after the treatment is high, when bare balloon dilatation is used for the treatment of intravascular stenosis. If bare metal stent catheter intervention is used for the treatment of intravascular stenosis, although the restenosis does not occur easily after the treatment, it may easily cause neointimal proliferation (i.e., the formation of scar tissue at a stent section). Once the restenosis or the neointimal proliferation occurs, another intervention treatment is required.
The objective of the present disclosure is to provide a balloon catheter system with repair function, which can solve the problems of restenosis and neointimal hyperplasia after the interventional treatment.

The balloon catheter system provided by the present disclosure concentrates the tearing of the vascular wall in a predetermined area and provides the laser having a specific wavelength to effectively repair the tearing in the vascular wall in the predetermined area, which can effectively avoid the occurrence of restenosis. When a vascular stent is implanted, the balloon catheter system of the present disclosure can also avoid neointimal proliferation.

As shown in
a tube body 130 having a distal end and a proximal end opposite to the distal end, and at least forming a fluid channel and an optical fiber channel;
a balloon body 110 located at an outer periphery of the distal end of the tube body 130 and communicating with the fluid channel;
at least one optical fiber 120 arranged in the optical fiber channel and having a light emitting section extending to a position of the balloon body 110; and
a laser generator connected to a proximal end of the optical fiber 120 and emitting laser having a repair function, wherein a wavelength of the laser ranges from 400 to 750 nm.
When the balloon catheter system 100 with repair function is used, firstly, fluid is injected into the balloon body 110 through the fluid channel of the tube body 130 to fill the balloon body 110. Then, a laser with a wavelength ranging from 400 to 750 nm is emitted by the laser generator. The laser is emitted from the light emitting section of the optical fiber 120 and is applied to a site to be repaired after passing through the balloon body 110 and the fluid inside the balloon body 110 and being scattered by the balloon body 110. Generally, continuous application of the laser for a predetermined period of time can achieve an ideal treatment effect.
High refractive index ultraviolet adhesive is attached to an outer periphery of the balloon body 110 and distributed at intervals. The high refractive index ultraviolet adhesive is regularly distributed on an outer surface of the balloon body 110. When the balloon body 110 is filled with fluid, the balloon body 110 expands to dilate the vascular wall of the human body. During the dilation of the vascular wall, a local tearing of the structure of the vascular wall may occur to achieve the dilation of the vascular wall. Since the refractive index ultraviolet adhesive protrudes from the outer surface of the balloon body 110, the tearing of the vascular wall is concentrated in areas where the high refractive index ultraviolet adhesive is distributed; when the high refractive index ultraviolet adhesive acts on the vascular wall, a stress concentration area in the vascular wall is more easily formed. By means of the arrangement of the high refractive index ultraviolet adhesive, the tearing of the vascular wall which is originally in disordered distribution is concentrated in the areas where the high refractive index ultraviolet adhesive is distributed. Meanwhile, in these areas, due to the enrichment effect of the high refractive index ultraviolet adhesive on the laser, the laser acts on the tearing area of the vascular wall more intensively, thereby completing the repairing of the tearing area.
With the high refractive index ultraviolet adhesive protruding from the outer surface of the balloon body 110, the technical solution of the present disclosure can concentrate the inevitably-disordered tearing of the vascular wall in specific areas where the high refractive index ultraviolet adhesive is distributed. At the same time, based on the enrichment effect of the high refractive index ultraviolet adhesive on the laser in the specific areas, the tearing of the vascular wall can be repaired more effectively.
The laser acts on the site to be repaired, which can promote the repairing of an injured blood vessel and effectively prevent the occurrence of restenosis.
The laser generator emits low-power laser in pulses or continuously emits the low-power laser, which is conducted into the balloon body 110 through the optical fiber 120. The light emitting section of the optical fiber 120 located inside the balloon body 110 applies the low-intensity laser to the site to be repaired. The low-intensity laser has a photo-modulating effect, which is generated by a photochemical effect rather than a thermal effect, thus, the low-intensity laser can promote the repairing of cells and tissues, accelerate the clearance of inflammatory mediators, and promote the absorption of tissue edema.
After the balloon catheter dilation, the inner wall of the blood vessel is easily injured. The red light of a predetermined wavelength emitted by the light emitting section of the optical fiber 120, which is applied to the injury site of the inner wall of the blood vessel after being scattered by the balloon body 110, can promote vascular endothelial cell proliferation and anti-inflammatory apoptosis, accelerate the blood vessel to return to a stable state, and prevent vascular restenosis after the balloon catheter dilation.
The laser emitted by the laser generator has a wavelength ranging from 400 to 750 nm. The laser appears red within the wavelength range, which can promote the repairing of cell nuclear tissues, and accelerate the clearance of inflammatory mediators and the absorption of tissue edema. In some embodiments, the laser emitted by the laser generator has a wavelength ranging from 630 to 700 nm.
The laser emitted by the laser generator has a power ranging from 10 to 30 mW. In some embodiments, the laser emitted by the laser generator has a power ranging from 10 to 15 mW.
The laser having a predetermined wavelength is emitted from the laser generator in the following sequence in each working cycle:
Each working cycle corresponds to a complete cycle in which the laser generator acts on the vascular wall. In the initial stage, the laser having a low power and lasting for a short duration is used to allow the blood vessel to adapt to the laser irradiation, preparing for the subsequent treatment. In the intermediate stage, the power of the laser is increased to heal the torn blood vessel, preventing restenosis after the dilation. In the final stage, the laser having a low power and lasting for a short duration is used for postoperative buffering, reducing the burden on the blood vessel.
If the power of the laser emitted by the laser generator is too low, the repairing result is unsatisfactory, while if the power of the laser is too high, vascular burns may be caused due to excessive heat. Therefore, in some embodiments, the laser has a power ranging from 10 to 15 mW/cm2 and lasts for 120 to 180 s during the intermediate stage.
The balloon body 110 can be a smooth bare balloon or a balloon with a different shape. The material of the balloon body 110 needs to have sufficient flexibility and good through-ability such that the balloon body 110 can smoothly reach an intended site. The balloon body 110 also needs to have an excellent processing performance, a good resilience, a fatigue resistance, and a dimensional stability. In some embodiments, the material of the balloon body 110 is one selected from PEBAX, nylon, and TPU. In some further embodiments, the material of the balloon body is PEBAX or nylon.

The black coating covered on the distal end of balloon body 110 can serve as a barrier to block laser radiation, preventing the laser from causing burns to unintended sites.

The laser emitted from the light emitting section is focused in the area corresponding to the high refractive index ultraviolet adhesive to ensure that the laser applied to the site to be repaired has a sufficient intensity. In some embodiments, a refractive index of the high refractive index ultraviolet adhesive ranges from 1.50 to 1.70. In some further embodiments, the refractive index of the high refractive index ultraviolet adhesive ranges from 1.54 to 1.62.
The tearing of the vascular wall is concentrated in the intended site through the high refractive index ultraviolet adhesive. To avoid complicating tearing of the vascular wall, the high refractive index ultraviolet adhesive is uniformly distributed on the outer periphery of the balloon body 110. The high refractive index ultraviolet adhesive protrudes from the outer surface of the balloon body 110, with a height ranging from 0.20 to 0.25 mm.

When the balloon body 110 is filled with fluid through the fluid channel of the tube body 130, the high refractive index ultraviolet adhesive can focus the laser, and the focused laser is applied more intensively to the site to be repaired, avoiding omission of the treatment area.
The high refractive index ultraviolet adhesive not only can concentrate the tearing area of the blood vessel and focus the laser for effective repairing, but also can increase a friction between the balloon body 110 and the vascular wall during the balloon body dilation, thereby reducing the slidable movement of the balloon body 110 relative to the vascular wall, improving the treatment effect, and reducing unnecessary injury to non-treatment areas.
The high refractive index ultraviolet adhesive in a regular distribution can also cut the lesion site, reducing the irregular tearing and dissection of the blood vessel.

The inner tube 132 of the tube body 130 has a hardness ranging from 35 to 75 D, and the hardness increases continuously or at intervals from the distal end of the tube body 130 to the proximal end of the tube body 130 by a predetermined value.
The inner tube 132 of the tube body 130 includes at least two first unit sections connected in sequence along an axial direction of the tube body 130. For two adjacent first unit sections, a hardness of the first unit section closer to the distal end of the tube body 130 is less than that of the first unit section closer to the proximal end of the tube body 130. In some embodiments, the proximal inner tube adopts a three-layer tube, and the distal inner tube adopts a three-layer tube or a single-layer tube.

In the present disclosure, by selecting appropriate material for the balloon body 110, the inner tube 132 and the outer tube 131, the balloon body 110, the inner tube 132, and the outer tube 131 can form a delivery system that is relatively more flexible, which can balance the increase in a hardness of the delivery system due to the addition of the optical fiber, such that the delivery system can have an ideal combination of radial support and flexibility.

At the distal end of the tube body 130, the inner tube 132 intersects with the distal end of the balloon body 110, and the distal end of the inner tube 132 is welded with the elastic member 170. The elastic member 170 may also be welded to the distal end of the balloon body 110. The elastic member 170 is generally located at the distal end of the inner tube 132. The welding section 172 overlaps with the distal end of the inner tube 132 and is connected to the distal end of the inner tube 132 by welding. The bare section 171 is directly exposed to the external environment, neither located in a wall of the inner tube 132 nor overlapping with other components.
The elastic member 170 has a certain elasticity. As the part of the tube body 130 that firstly extends into the blood vessel, the elastic member 170 is deformable when encountering an obstruction inside the blood vessel, which can avoid puncturing the blood vessel. Meanwhile, based on the resilience of the elastic member 170, the elastic member 170 can restore to its original shape after the external force is removed.
The elastic member 170 is a spiral spring, which is formed by spirally winding of a metal wire having a diameter ranging from 0.04 to 0.1 mm. In some embodiments, the spiral spring is formed by spirally winding of a metal wire with a diameter ranging from 0.05 to 0.06 mm. The metal wire is made of one selected from platinum-tungsten, platinum-iridium, stainless steel, gold, and nickel-titanium.
The elastic member 170 is made of metal wires, which is opaque, thus, the elastic member 170 can shield the light emitted by the optical fiber at the distal end of the balloon body 110 to avoid injury to unintended sites due to the laser radiation.
A thread pitch of the bare section 171 is equal to a diameter of the metal wire, and a thread pitch of the welding section 172 is 1 to 3 times the diameter of the metal wire. In some embodiments, the thread pitch of the welding section 172 is 1.5 to 2 times the diameter of the metal wire.
The bare section 171 is exposed to the external environment. Since the bare section 171 has a relatively-smaller thread pitch, the bare section 171 can improve the resilience of the elastic member 170. However, since the welding section 172 needs to be connected to the wall of the inner tube 132, the thread pitch of the welding section 172 needs to be set to be relatively larger, such that the welding section 172 can avoid the situation that the flexibility of the wall of the inner tube 132 is excessively reduced, which may easily cause injury to the vascular wall.
The metal wire has a developing function, and the elastic member 170 can also serve as a developing component to indicate the position of the balloon body 110 in the human body.
An axial length of the elastic member 170 ranges from 2 to 5 mm. In some embodiments, the axial length of the elastic member 170 ranges from 3 to 5 mm.
The axial length of the elastic member 170 needs to be appropriate. A too-long axial length is adverse to the restoring of the elastic member 170, and a too-short axial length may prevent the elastic member 170 from playing a role in guiding the balloon body 110 forwards along the blood vessel.
The axial length of the bare section 171 of the elastic member 170 ranges from 0.5 to 2 mm, and the axial length of the welding section 172 ranges from 1 to 3 mm. In some embodiments, the axial length of the bare section 171 ranges from 1 to 2 mm, and the axial length of the welding section 172 ranges from 2 to 3 mm.
A diameter of the bare section 171 ranges from 0.50 to 0.70 mm, and a diameter of the welding section 172 ranges from 0.60 to 1.0 mm. In some embodiments, the diameter of the bare section 171 ranges from 0.5 to 0.6 mm, and the diameter of the welding section 172 ranges from 0.6 to 0.7 mm.
The diameter of the bare section 171 needs to allow the bare section 171 to enter a thin blood vessel without losing the resilience of the bare section 171, and the diameter of the welding section 172 matches with that of the inner tube 132.
A ratio of a diameter of the distal end of the bare section 171 to that of the proximal end of the welding section 172 ranges from 1:1 to 1:1.5. In some embodiments, the ratio ranges from 1:1 to 1:1.2.
The proximal end of the welding section 172 forms a welding ring 173 welded to the wall of the inner tube 132 by bending the metal wire forming the spiral spring. The welding ring 173 can increase a welding area and ensure the firmness of the welding connection.

The optical fiber 120 can be made of plastic or quartz. In some embodiments, the optical fiber 120 is made of plastic. A diameter of the optical fiber 120 ranges from 0.125 to 0.25 mm; in some embodiments, the diameter of the optical fiber 120 is 0.125 mm.

The light emitting section of the optical fiber 120 is produced by removing a cladding from a portion of the optical fiber 120 located within the balloon body 110, and a length of the removed cladding is equal to the axial length of the balloon body 110. The guide wire is guided to extend out of balloon body 110 through the double-lumen tube 133, while the optical fiber 120 is placed inside the lumen of the inner tube 132 which originally forms a guide wire channel. The optical fiber 120 and the inner tube 132 are coaxially arranged (within an acceptable margin of error, and it is not necessary to require that the optical fiber 120 and the inner tube 132 are strictly coaxial), which allows the light emitted by the optical fiber to illuminate the vascular wall more uniformly.
The path of the guide wire 180 is partially located outside balloon body 110, thus, the lesion site can be cut to reduce irregular tearing and dissections of the blood vessel.
To improve the treatment effect, the outer surface of the balloon body 110 is coated with at least one selected from an anti-proliferative drug, a drug that induce collagen or elastin cross-linking, and an anti-vasospasm drug.
The drug coating on the outer surface of the balloon body 110 can act on the injury site, which can achieve corresponding efficacy and facilitate comprehensive repairing of the injury site. In some embodiments, the drug is paclitaxel or rapamycin.
In some embodiments, the outer surface of balloon body 110 is provided with an implantable vascular stent.
When being used to provide laser repairing, the balloon body 110 can be used to implant the vascular stent into the human body at the same time, achieving two treatment objectives at once. The vascular stent generally has a compressible mesh structures that does not significantly block the light during the laser repairing process, thus, the effect of the laser repairing may not be affected by the vascular stent. Moreover, based on the result of the laser repairing, after the vascular stent is implanted, problems like neointimal hyperplasia may not be easily caused.
The technical features of the above embodiments can be combined in any way. To simplify the description, not all possible combinations of the technical features in the above embodiments have been described. However, as long as the combination of these technical features does not contradict each other, it should be considered within the scope of this specification.
