Detection and treatment of cancers of the liver

Abstract

The present invention relates to compositions and methods for cancer therapies and diagnostics, including but not limited to, cancer markers. In particular, the present invention provides tumor antigens associated with specific cancers and diagnostic assays for the detection of such antigens and associated autoantibodies as indicative of the presence of specific cancers. The present invention further provides cancer immunotherapy utilizing the tumor antigens of the present invention.

Description

FIELD OF THE INVENTION

[0003] The present invention relates to compositions and methods for cancer therapies and diagnostics, including but not limited to, cancer markers. In particular, the present invention provides tumor antigens associated with specific cancers and diagnostic assays for the detection of such antigens and associated autoantibodies as indicative of the presence of specific cancers.

BACKGROUND OF THE INVENTION

[0004] The term cancer collectively refers to more than 100 different diseases that affect nearly every part of the body. Throughout life, healthy cells in the body divide, grow, and replace themselves in a controlled fashion. Cancer starts when the genes directing this cellular division malfunction, and cells begin to multiply and grow out of control. A mass or clump of these abnormal cells is called a tumor. Not all tumors are cancerous. Benign tumors, such as moles, stop growing and do not spread to other parts of the body. But cancerous, or malignant, tumors continue to grow, crowding out healthy cells, interfering with body functions, and drawing nutrients away from body tissues. Malignant tumors can spread to other parts of the body through a process called metastasis. Cells from the original tumor break off, travel through the blood or lymphatic vessels or within the chest, abdomen or pelvis, depending on the tumor, and eventually form new tumors elsewhere in the body.

[0005] Only 5-10% of cancers are thought to be hereditary. The rest of the time, the genetic mutation that leads to the disease is brought on by other factors. The most common cancers are linked to smoking, sun exposure, and diet. These factors, combined with age, family history, and overall health, contribute to an individual's cancer risk.

[0006] Several diagnostic tests are used to rule out or confirm cancer. For many cancers, the most definitive way to do this is to take a small sample of the suspect tissue and look at it under a microscope—this process is called a biopsy. However, many biopsies are invasive, unpleasant procedures with their own associated risks, such as pain, bleeding, infection, and tissue or organ damage. In addition, if a biopsy does not result in an accurate or large enough sample, a false negative or misdiagnosis can result, often required that the biopsy be repeated. What is needed in the art are improved methods to specifically detect, characterize, and monitor specific types of cancer.

SUMMARY OF THE INVENTION

[0007] The present invention relates to compositions and methods for cancer therapies and diagnostics, including but not limited to, cancer markers. In particular, the present invention provides tumor antigens associated with specific cancers and diagnostic assays for the detection of such antigens and associated autoantibodies as indicative of the presence of specific cancers.

[0008] There is increasing evidence for an immune response to cancer in humans, demonstrated by the identification of autoantibodies to tumor antigens (Stocket et al., J. Exp. Med., 187:1349 [1998]; Boon and Old, Curr. Opin. Immunol. 9:681 [1997]; Soussl, Cancer Res. 60:1777 [2000]; Old and Chen, J. Exp. Med. 187:1163 [1998]). The identification of panels of tumor antigens that elicit a humoral response has utility in cancer screening, diagnosis, and in establishing a prognosis. Such antigens also have utility in immunotherapy against cancers. Several approaches are currently available for the identification of tumor antigens. The present invention provides a proteomic-based approach for the identification of tumor antigens that induce an antibody response. In contrast to other approaches based on the analysis of recombinant proteins, a proteomic approach allows identification of autoantibodies to proteins that are directly derived from cancer cells or tumors and thus may uncover antigenicity associated with post-translational modification.

[0009] Antigens that have been shown to induce a humoral response in HCC include p53 (Reedle et al., Eut. J. Cancer, 34:1198 [1998]; Saffroy et al., Br. J. Cancer, 79:604 [1999]), the nuclear proteins HCC1 (Imai et al., J. Clin. Invest., 92:2419 [1993]), SG2DNA (Muro et al., Biochem. Biophys. Res. Commun., 207:1029 [1995]), NOR-90, fibrillarin, nucleophosmin/B23 and p330d/CENP-F (Imai et al., Am. J. Pathol., 140:859 [1992]; Covini et al., J. Hepatol., 26:1255 [1997]). Autoantibodies to DNA Topoisomerase II (Imai et al., Clin Cancer Res., 1:417 [1995]), to cyclin B1 (Covini et al., Hepatology, 25:75 [1997]) and to a novel cytoplasmic protein with RNA-binding motifs (Zhang et al., J. Exp. Med., 189:1101 [1999]) have also been reported. A SEREX (serological analysis of recombinant cDNA expression libraries) study of hepatocellular carcinoma has uncovered reactivity to diverse proteins involved in the transcription/translational machinery as well as to chaperone proteins (Stenner-Llewen et al., Cancer Epidemiol. Biomarkers Prev., 9:285 [2000]).

[0010] Experiments performed during the development of the present invention identified 8 proteins that elicited a humoral response with specificity to HCC, based on analysis of 37 patients with HCC and additional controls. A cleavage product of calreticulin was found to commonly induce autoantibodies in hepatocellular carcinomas. Thus, the present invention provides additional markers for use in the diagnosis and treatment of HCC.

[0011] Accordingly, in some embodiments, the present invention provides a method for detecting cancer, comprising providing a sample from a subject suspected of having cancer; and detecting the presence of a tumor antigen in the sample, thereby detecting cancer. In some embodiments, the tumor antigen is Crt32. In some embodiments, the cancer is hepatocellular carcinoma. In some embodiments, the subject comprises a human subject. In some embodiments, the sample is selected from the group including, but not limited to, a blood sample and a tumor sample. In some embodiments, detecting comprises exposing the sample to an antibody and detecting the antibody binding to the tumor antigen. In other embodiments, detecting comprises detecting the presence of an autoantibody to the tumor antigen. In some embodiments, detecting comprises exposing the sample to an autoantibody specific antibody and detecting the autoantibody specific antibody binding to the antibody. In some embodiments, the method further comprises the step of providing a prognosis to the subject. In certain embodiments, detecting cancer further comprises detecting a stage of the cancer. In other embodiments, detecting cancer further comprises detecting a sub-type of the cancer.

[0012] The present invention further provides a kit for detecting the presence of cancer in a subject, comprising a reagent capable of specifically detecting the presence of a tumor antigen; and instructions for using the reagent for detecting the presence of cancer in the subject. In some embodiments, the antibody is a tumor antigen specific antibody. In certain embodiments, the antibody is an antibody specific for an autoantibody to the tumor antigen. In some embodiments, the tumor antigen is Crt32.

[0013] The present invention additionally provides a method for eliciting a cancer specific immune response, comprising providing an immunogenic composition comprising a tumor antigen; and a subject diagnosed with a cancer; and administering the immunogenic composition to the subject under conditions that the subject generates an immune response to the cancer. In some embodiments, the tumor antigen is Crt32. In some embodiments, the immunogenic composition further comprises an immune enhancing cytokine. In some embodiments, the immune enhancing cytokine is expressed by a cell. In some preferred embodiments, the immune response results in a detectable decrease in the presence of the cancer. In certain embodiments, the immune response results in a measurable decrease in the level of the tumor antigen. In some embodiments, the immune response results in a measurable decrease in the level of autoantibodies to the tumor antigen. In some embodiments, the cancer is hepatocellular carcinoma. In certain embodiments, the subject is a human.

[0014] The present invention also provides a method of treating cancer in a subject, comprising providing a subject; and a therapeutic composition comprising an antibody directed toward a tumor antigen; and administering the therapeutic composition to the subject. In some embodiments, the tumor antigen is Crt32. In some embodiments, the cancer is hepatocellular carcinoma. In some embodiments, the antibody is attached to a cytotoxic agent. In some embodiments, the cytotoxic agent is selected from the group including, but not limited to, chemotherapeutic agents, radioisotopes, cytosines, cytokines, and toxins. In some embodiments, the cytotoxic agent is Ricin A chain.

[0017] The term “epitope” as used herein refers to that portion of an antigen that makes contact with a particular antibody.

[0018] When a protein or fragment of a protein is used to immunize a host animal, numerous regions of the protein may induce the production of antibodies which bind specifically to a given region or three-dimensional structure on the protein; these regions or structures are referred to as “antigenic determinants”. An antigenic determinant may compete with the intact antigen (i.e., the “immunogen” used to elicit the immune response) for binding to an antibody.

[0019] The terms “specific binding” or “specifically binding” when used in reference to the interaction of an antibody and a protein or peptide means that the interaction is dependent upon the presence of a particular structure (i.e., the antigenic determinant or epitope) on the protein; in other words the antibody is recognizing and binding to a specific protein structure rather than to proteins in general. For example, if an antibody is specific for epitope “A,” the presence of a protein containing epitope A (or free, unlabelled A) in a reaction containing labeled “A” and the antibody will reduce the amount of labeled A bound to the antibody.

[0020] As used herein, the terms “non-specific binding” and “background binding” when used in reference to the interaction of an antibody and a protein or peptide refer to an interaction that is not dependent on the presence of a particular structure (i.e., the antibody is binding to proteins in general rather that a particular structure such as an epitope).

The protein may be expressed by non tumor cells but be immunogenic only when expressed by a tumor cell. Alternatively, the protein may be expressed by tumor cells, but not normal cells.

However, a “subject suspected of having cancer” encompasses an individual who has received an initial diagnosis (e.g., a CT scan showing a mass) but for whom the sub-type or stage of cancer is not known. The term further includes people who once had cancer (e.g., an individual in remission).

Introns are removed or “spliced out” from the nuclear or primary transcript; introns therefore are absent in the messenger RNA (mRNA) transcript. The mRNA functions during translation to specify the sequence or order of amino acids in a nascent polypeptide.

[0041] As used herein, the term “heterologous gene” refers to a gene that is not in its natural environment. For example, a heterologous gene includes a gene from one species introduced into another species. The promoter and enhancer elements that direct transcription of a linked gene are generally located 5′ or upstream of the coding region. However, enhancer elements can exert their effect even when located 3′ of the promoter element or the coding region. Transcription termination and polyadenylation signals are located 3′ or downstream of the coding region.

[0047] As used herein, the terms “an oligonucleotide having a nucleotide sequence encoding a gene” and “polynucleotide having a nucleotide sequence encoding a gene,” means a nucleic acid sequence comprising the coding region of a gene or in other words the nucleic acid sequence that encodes a gene product. The coding region may be present in a cDNA, genomic DNA or RNA form. When present in a DNA form, the oligonucleotide or polynucleotide may be single-stranded (i.e., the sense strand) or double-stranded. Suitable control elements such as enhancers/promoters, splice junctions, polyadenylation signals, etc. may be placed in close proximity to the coding region of the gene if needed to permit proper initiation of transcription and/or correct processing of the primary RNA transcript. Alternatively, the coding region utilized in the expression vectors of the present invention may contain endogenous enhancers/promoters, splice junctions, intervening sequences, polyadenylation signals, etc. or a combination of both endogenous and exogenous control elements.

[0048] As used herein, the term “oligonucleotide,” refers to a short length of single-stranded polynucleotide chain. Oligonucleotides are typically less than 200 residues long (e.g., between 15 and 100), however, as used herein, the term is also intended to encompass longer polynucleotide chains. Oligonucleotides are often referred to by their length. For example a 24 residue oligonucleotide is referred to as a “24-mer”. Oligonucleotides can form secondary and tertiary structures by self-hybridizing or by hybridizing to other polynucleotides. Such structures can include, but are not limited to, duplexes, hairpins, cruciforms, bends, and triplexes.

[0049] As used herein, the term “regulatory element” refers to a genetic element that controls some aspect of the expression of nucleic acid sequences. For example, a promoter is a regulatory element that facilitates the initiation of transcription of an operably linked coding region. Other regulatory elements are splicing signals, polyadenylation signals, termination signals, etc. (defined infra).

[0050] Transcriptional control signals in eukaryotes comprise “promoter” and “enhancer” elements. Promoters and enhancers consist of short arrays of DNA sequences that interact specifically with cellular proteins involved in transcription (T. Maniatis et al., Science 236:1237 [1987]). Promoter and enhancer elements have been isolated from a variety of eukaryotic sources including genes in yeast, insect and mammalian cells, and viruses (analogous control elements, i.e., promoters, are also found in prokaryote). The selection of a particular promoter and enhancer depends on what cell type is to be used to express the protein of interest. Some eukaryotic promoters and enhancers have a broad host range while others are functional in a limited subset of cell types (for review, See e.g., Voss et al., Trends Biochem. Sci., 11:287 [1986]; and T. Maniatis et al., supra). For example, the SV40 early gene enhancer is very active in a wide variety of cell types from many mammalian species and has been widely used for the expression of proteins in mammalian cells (Dijkema et al., EMBO J. 4:761 [1985]). Two other examples of promoter/enhancer elements active in a broad range of mammalian cell types are those from the human elongation factor 1α gene (Uetsuki et al., J. Biol. Chem., 264:5791 [1989]; Kim et al., Gene 91:217 [1990]; and Mizushima and Nagata, Nuc. Acids. Res., 18:5322 [1990]) and the long terminal repeats of the Rous sarcoma virus (Gorman et al., Proc. Natl. Acad. Sci. USA 79:6777 [1982]) and the human cytomegalovirus (Boshart et al., Cell 41:521 [1985]). Some promoter elements serve to direct gene expression in a tissue-specific manner.

[0051] As used herein, the term “promoter/enhancer” denotes a segment of DNA which contains sequences capable of providing both promoter and enhancer functions (i.e., the functions provided by a promoter element and an enhancer element, see above for a discussion of these functions). For example, the long terminal repeats of retroviruses contain both promoter and enhancer functions. The enhancer/promoter may be “endogenous” or “exogenous” or “heterologous.” An “endogenous” enhancer/promoter is one that is naturally linked with a given gene in the genome. In particular, the present invention provides tumor antigens associated with specific cancers and diagnostic assays for the detection of such antigens and associated autoantibodies as indicative of the presence of specific cancers.

[0105] Evidence for an increase in the number of cases of hepatocellular carcinoma (HCC) over the past two decades has been reported (EI-Serag and Mason, New Eng. J. Med. 340:745 [1999]). Most cases of hepatocellular carcinoma are associated with cirrhosis (at least 90% in America and Europe) (Ikeda et al., Hepatology 18:47 [1993]). The risk for HCC increases by a factor of approximately 200 with the onset of cirrhosis and up to 30-50% of patients with cirrhosis develop a hepatocellular carcinoma within ten years. These tumors have a poor prognosis, with five-year survival rates of less than 5 percent. Chronic infections with hepatitis B (HBV) and C(HCV) viruses are major risk factors for HCC. Development of a chronic carrier state is a frequent event following acute viral infection (Brechot et al., J. Hepatol. 29:173 [1998]; Ikeda et al., J. Hepatol. 28:930 [1998]; Moradpour et al., Hepatic Oncogenesis in Hepatology: A textbook of liver disease. D. Zakim and TD Boyer (eds) Philadelphia: W B Sanders 1490-1512 [1996]; Heintges et al., Hepatology 26:521 [1997]). The most likely explanation for the rising incidence of HCC is the spread of hepatitis virus in the population.

[0106] Symptoms of HCC include abdominal pain or tenderness, particularly in the right-upper quadrant and an enlarged, tender liver. HCC is diagnosed by liver biopsy and/or a mass shown on an abdominal CT scan. In addition, a liver scan may indicate an abnormality or liver enzymes (liver function tests) are elevated. The biologic marker Serum alpha-fetoprotein (AFP) is also used for the diagnosis of HCC. By a radioimmunoassay technique, 50% to 70% of patients in the United States who have hepatocellular carcinoma have elevated levels of AFP. However, patients with other malignancies (germ cell carcinoma and, rarely, pancreatic and gastric carcinoma) also demonstrate elevated serum levels of this protein, thus decreasing the utility of AFP as a screening marker. AFP levels have been shown to be prognostically important, with the median survival of AFP-negative patients significantly longer than that of AFP-positive patients (Stillwagon et al., International Journal of Radiation Oncology, Biology, Physics 20: 65 [1991]; Izumi et al., Journal of Surgical Oncology 49:151 [1992]). Other prognostic variables include performance status and liver functions (Yamashita et al., Cancer 67: 385 [1991]).

[0107] Prognosis depends on the degree of local tumor replacement and the extent of liver function impairment. However, the probable outcome of HCC is poor. The course of the disease is rapid and usually fatal. Most patients die within 3 to 6 months. Complications include gastrointestinal hemorrhage, liver failure, and spread (metastasis) of the carcinoma.

[0108] The most common form of treatment of HCC is surgery or liver transplantation. Aggressive surgery or liver transplantation may be successful in treating small or slow-growing tumors if they are diagnosed early. Therapy other than surgical resection is generally administered as part of a clinical trial. Such trials evaluate the efficacy of systemic or infusional chemotherapy, hepatic artery ligation or embolization, and radiolabeled antibodies, often in conjunction with surgical resection and/or radiation therapy. Laparoscopic evaluation may detect metastatic disease, bilobar disease, or inadequate liver remnant, and therefore obviate the need for open surgical exploration.

[0115] In some embodiments, proteins from non-cancerous and cancerous cells (and/or tissues) are separated using an established two-dimensional (2-D) PAGE procedure (See e.g., Strahler et al., 1989. Protein Structure: A practical approach, T. E. Creighton ed., IRL Press, England, pgs. 65-92). Briefly, cells and tissues are solubilized in lysis buffer containing carrier ampholytes. Proteins are then applied to isoelectric focusing gels and separated based on isoelectric point. The first-dimension gel is then loaded onto the second dimension gel (acrylamide gradient). Proteins are then transferred to a PVDF membrane for Western blotting or visualized by silver-staining of the acrylamide gradient gels. In some embodiments, proteins separated by 2-D PAGE are characterized using Western blotting. Following transfer to PVDF membranes, the membranes are incubated with serum obtained from patients or from controls and bound antibodies are visualized.

These tumor antigens, in particular Crt32, find utility in the diagnosis and characterization of HCC, as described below.

These antibodies find use in the diagnostic and therapeutic methods described below.

The overall effect of such interference with target nucleic acid function is modulation of the expression of tumor antigens. In the context of the present invention, “modulation” means either an increase (stimulation) or a decrease (inhibition) in the expression of a gene. For example, expression may be inhibited to potentially prevent tumor proliferation or stimulated to increase a cancer-specific immune response (e.g., as a cancer vaccine).

No. 4,873,191 describes a method for the micro-injection of zygotes; the disclosure of this patent is incorporated herein in its entirety.

In some embodiments, the retroviral vector is utilized to transfect oocytes by injecting the retroviral vector into the perivitelline space of the oocyte (U.S. Pat. Transfection is easily and efficiently obtained by culturing the blastomeres on a monolayer of virus-producing cells (Stewart, et al., EMBO J., 6:383 [1987]). Alternatively, infection can be performed at a later stage. Virus or virus-producing cells can be injected into the blastocoele (Jahner et al., Nature 298:623 [1982]). Most of the founders will be mosaic for the transgene since incorporation occurs only in a subset of cells that form the transgenic animal. Further, the founder may contain various retroviral insertions of the transgene at different positions in the genome that generally will segregate in the offspring. In addition, it is also possible to introduce transgenes into the germline, albeit with low efficiency, by intrauterine retroviral infection of the midgestation embryo (Jahner et al., supra [1982]). Additional means of using retroviruses or retroviral vectors to create transgenic animals known to the art involve the micro-injection of retroviral particles or mitomycin C-treated cells producing retrovirus into the perivitelline space of fertilized eggs or early embryos (PCT International Application WO 90/08832 [1990], and Haskell and Bowen, Mol. Reprod. Dev., 40:386 [1995]).

Alternatively, the polymerase chain reaction may be used to screen for ES cells that have integrated the transgene. This technique obviates the need for growth of the transfected ES cells under appropriate selective conditions prior to transfer into the blastocoel.

[0223] Tissues and Cell Lines

[0224] Sera were obtained at the time of diagnosis from 37 patients with HCC, following informed consent. This group consisted of 30 males and 7 females with an age range of 20 to 86 (mean=59 years). Clinical information regarding the 37 patients analyzed is shown in Table 1. HBV and HCV status was determined by combining serological assays for HBsAg and anti-HCV plus PCR for HBV DNA and HCV RNA. Sera from 116 patients with other cancers (consisting of 24 with breast cancer, 62 with lung cancer, 16 with brain tumors, 7 with melanoma and 17 with esophageal cancer), from 21 subjects infected with HBV and 10 infected with HCV, from 3 patients with active systemic lupus erythematosus (SLE), and from 24 healthy individuals were utilized as controls. Tumor and non-tumor counterpart tissues were obtained from patients with HCC. Following excision, the tissues were immediately frozen at −80° C. The human hepatoma cell lines PLC-PRF6 and Huh7 were cultured in DMEM supplemented with 10% fetal calf serum, 100 U/ml penicillin and 100 U/ml streptomycin (GIBCO-BRL, Grand Island, Ky.).

Cultured cells, tumor and non-tumor tissues were solubilized in lysis buffer containing 9.6 M urea (Bio-Rad, Rockville Center, N.Y.), 2% nonidet P-40, 2% carrier ampholytes pH 4-8 (Gallard/Schlessinger, Carte Place, N.Y.), 2% β-mercaptoethanol and 10 mM phenylmethanesulfonyl fluoride (PMSF). Protein concentration was measured by Bradford assay (Bio-Rad, Hercules, Calif.). Proteins (175 μg) were applied onto isofocusing gels. Isoelectric focusing was conducted using pH 4 to 8 carrier ampholytes at 700 V for 16 hrs, followed by 1000 V for an additional 2 hrs. The first-dimension gel was loaded onto a second dimension gel equilibrated in 125 mM Tris, pH 6.8, 10% glycerol, 2% SDS, 1% dithiothreitol and bromophenol blue.

[0229] Following transfer, membranes were incubated for 2 hr in blocking buffer containing 5% milk in 10 mM Tris-HCl pH 7.5, 2.5 mM EDTA pH 8; 50 mM NaCl. The membranes were incubated for 2 hr with serum obtained either from patients or from controls (1:300 dilution). After three washes, the membranes were incubated with horseradish peroxidase-conjugated anti-human IgG antibody (1:1000 dilution) (Amersham, Arlington Heights, Ill.). Immunodetection was accomplished by enhanced chemiluminescence (ECL) (Amersham) followed by autoradiography on hyperfilm MP (Amersham). Immunoblotting assays for the detection of calreticulin were performed by using antibodies against calreticulin: SPA-600 (StressGen, Victoria, Canada.) and T-16 (Santa Cruz Biotechnology, Santa Cruz, CA) at a dilution of 1:1,000,000, 1:10,000, respectively. The membranes were then incubated for 1 hr with horseradish peroxidase-conjugated anti-rabbit (Amersham) or anti-goat (Sigma) IgG antibodies at a dilution of 1:1000.

Example 4

[0230] In-Gel Enzymatic Digestion and Mass Spectrometry

[0231] 2-D gels were silver stained by successive incubations in 0.02% sodium thiosulfate for 2 min, 0.1% silver nitrate for 40 mins and 0.014% formaldehyde plus 2% sodium carbonate. The proteins of interest were excised from the 2-D gels and destained for 5 min in 15 mM potassium ferricyanide and 60 mM sodium thiosulfate as described (Gharahdaghi et al., Electrophoresis, 20:601 [1999]). Following 3 washes with water; the gel pieces were dehydrated in 100% acetonitrile for 5 min and dried for 30 min in a vacuum centrifuge. Digestion was performed by addition of 100 ng of trypsin (Promega, Madison, Wis.) in 200 mM ammonium bicarbonate. Following enzymatic digestion overnight at 37° C., the peptides were extracted twice with 50 μl of 60% acetonitrile/1% trifluoro-acetic acid. After removal of acetonitrile by centrifugation in a vacuum centrifuge, the peptides were concentrated by using pipette tips C18 (Millipore, Bedford, Mass.). Analyses were performed primarily using a PerSeptive Biosystem matrix assisted laser desorption ionization-time of flight (MALDI-TOF) Voyager-DE mass spectrometer (Framingham, Mass.), operated in delayed extraction mode. Peptide mixtures were analyzed using a saturated solution of α-cyano-4-hydroxycinnamic acid (CHA) (Sigma) in acetone containing 1% trifluoro-acetic acid (Sigma). Peptides were selected in the mass range of 800-4000 Da. Spectra were calibrated using calibration mixture 2 of the Sequezyme peptide mass standards kit (PerSeptive Biosyetems, Framingham, MA). The search program MS-Fit, developed by the University of California at San Francisco (available at the Internet Web of University of California at San Francisco), was used for searches in the database NCBI. Search parameters were as follows: maximum allowed peptide mass error of 400 ppm, consideration of one incomplete cleavage per peptide and pH range between 4 and 8.

Example 5

[0232] Identification of Autoantibodies in HCC Patients

[0233] The presence of autoantibodies in sera from patients with HCC was investigated using two hepatoma cell lines as an antigen source. Proteins extracted from the two cell lines (PLC-PRF5 and Huh7) were separately subjected to two-dimensional gel electrophoresis (2-D PAGE) and transferred onto Immobilon-P membranes. Initially, sera from 37 patients with HCC (see Table 1) and from 24 healthy subjects were screened individually by Western blot analysis, in which each serum was incubated with one blot of PLC-PRFS and a separate blot of Huh7 separated proteins. A secondary anti-IgG antibody was utilized to detect reactive proteins. A greater number of reactive proteins were detected with sera from patients with HCC then with control sera. Proteins that exhibited selective reactivity with sera from HCC patients were targeted for further characterization. This set consisted of 13 proteins that exhibited reactivity with at least 4 (11%) of HCC sera (Table 2). Among the 13 proteins, 8 were common to both PLC-PRFS and Huh7 cell lines. Two reactive proteins were observed only in PLC-PRFS and three only in Huh7 extracts, reflecting differences in the protein expression profiles between the two cell lines. In all cases, sera that reacted to a protein in one cell line also reacted with the same protein in the other cell line, if detected in the silver stained 2-D patterns. In all, 27 of the 37 HCC sera (73%) showed reactivity against at least one of the 13 proteins, and 17 of the 37 sera showed reactivity against at least two of the 13 proteins (Table 2).

[0234] B. Identification of Reactive Proteins

[0235] For protein identification, additional 2-D gels were produced with the same Huh7 and PLC-PRF6 cellular extracts, and were silver stained as described above. The 13 proteins of interest were then excised from the gels, digested with trypsin and subsequently analyzed by MALDI-TOF mass spectrometry. The resulting spectra were used to identify the proteins using the MS-FIT search program. Of the 13 spots excised from the gels, 11 were identified (Table 3). The proteins identified were members of different groups including chaperones such as hsp60 and calreticulin, and structural proteins such as cytokeratin 8, cytokeratin 18 and beta tubulin. The protein in spot 12 with an estimated MW of 32 kDa and PI 4.1 was identified as corresponding to a calreticulin isoform by mass spectrometry. Calreticulin is a protein with a MW of 48 kDa and a PI of 4.3 and no forms of calreticulin with a MW of 3.2 kDa have been previously reported. Additional enzymatic digestions were performed and peptides that exhibited high intensities were further analyzed by ESI mass spectrometry in order to obtain the sequence of these peptides. Altogether, the results indicated that the protein in spot 12, designated Crt32, represented a cleavage product of calreticulin, corresponding to the C-terminal end (AA 157-400). Identification of the protein in spot 12 as the C-terminal portion of calreticulin was further confirmed by Western-blotting using two specific antibodies against calreticulin, SPA-600 and T-19 antibodies, produced against a C-terminal and a N-terminal peptide, respectively. The protein-spot reacted with the SPA-600 antibody but not with the T-19 antibody. These antibodies also confirmed the identification of spots 1, 2 and 5 as other calreticulin isoforms. All calreticulin isoforms, including Crt32, were the proteins with the highest frequency of autoantibodies (27%), in sera from patients with HCC.

[0236] C. Specificity of Calreticulin and Crt32 Autoantibodies in HCC

[0237] Calreticulin has been shown to be antigenic in autoimmune diseases such as systemic lupus erythematosus (SLE). The presence of autoantibodies has been reported in 4.0% of SLE patients and the epitopes eliciting a humoral response have been located in the N-terminal part of the molecule (Eggleton et al., Clin. Exp. Immunol., 120:384 [2000]). Therefore, the calreticulin autoreactivity in sera from HCC patients and from 3 SLE patients was investigated. Strong reactivity against full-length calreticulin but not against Crt32 was observed in SLE patients, indicating a difference in epitopes that induce autoreactivity between HCC and SLE patients, which are located in the C-terminal and N-terminal parts of the molecule, respectively.

[0238] Autoantibodies against calreticulin and Crt32 were largely restricted to liver cancer patients among the different cancer sera analyzed (Table 4). Crt32 autoantibodies were only found in sera of 2 of 24 patients with breast cancer and in 2 of 52 patients with lung cancer. None of the sera from other cancer types (16, 7, and 17 patients with brain cancer, melanoma and esophageal cancer, respectively) exhibited autoantibodies against these proteins.

[0239] Among the 10 patients with HCC presenting with autoantibodies against Crt32, five had cirrhosis, six were infected with HBV and one was infected with HCV. These results suggested that the occurrence of anti-Crt32 autoantibodies In HCC patients was related to liver cancer but not to cirrhosis, HBV or HCV infection as such. In order to evaluate the use of Crt32 autoantibodies as a marker of risk for HCC, the frequency of Crt32 autoantibodies in sera from 31 individuals infected with HBV or HCV (21 and 10, respectively) but not diagnosed with liver cancer was investigated. Reactivity against Crt32 was observed in 4 of these patients (13%) (Table 5), as compared to one of 24 healthy controls.

[0241] D. Crt32 Expression in Liver Tumors of Reactive Patients

[0242] The expression of Crt32 in liver tumor and non-tumor tissue in the same patients was investigated. Tissues from 4 patients with Crt32 autoantibodies (patients 6, 12, 25 and 28) were investigated. The expression level of Crt32 was measured by 2-D PAGE and sliver staining using a computerized quantitative approach as described above. Expression of Crt32 was elevated (3.9+1.3 fold; mean+SEM) in tumor as compared to non-tumor counterpart tissue. No variation of Crt32 levels was observed between tumor and non-tumor tissues obtained from anti-Crt32 autoantibodies of negative HCC patients. Up-regulation of Crt32 may contribute to the humoral response elicited against the C-terminal and of calreticulin in patients with HCC.

Claims

1. A method for detecting cancer, comprising: a) providing a sample from a subject suspected of having cancer; and b) detecting the presence of a tumor antigen in said sample, thereby detecting cancer.

2. The method of claim 1, wherein said tumor antigen is Crt32.

3. The method of claim 1, wherein said cancer is hepatocellular carcinoma.

4. The method of claim 1, wherein said subject comprises a human subject.

5. The method of claim 1, wherein said sample is selected from the group consisting of a blood sample and a tumor sample.

6. The method of claim 1, wherein said detecting comprises exposing said sample to an antibody and detecting said antibody binding to said tumor antigen.

7. The method of claim 1, wherein said detecting comprises detecting the presence of an autoantibody to said tumor antigen.

8. The method of claim 7, wherein said detecting comprises exposing said sample to an autoantibody specific antibody and detecting said autoantibody specific antibody binding to said antibody.

9. The method of claim 1, wherein said method further comprises step c) providing a prognosis to said subject.

10. The method of claim 1, wherein said detecting cancer further comprises detecting a stage of said cancer.

11. The method of claim 1, wherein said detecting cancer further comprises detecting a sub-type of said cancer.

12. A kit for detecting the presence of cancer in a subject, comprising: a) a reagent capable of specifically detecting the presence of a tumor antigen; and b) instructions for using said reagent for detecting the presence of cancer in said subject.

13. The kit of claim 12, wherein said antibody is a tumor antigen specific antibody.

14. The kit of claim 12, wherein said antibody is an antibody specific for an autoantibody to said tumor antigen.

15. The kit of claim 12, wherein said tumor antigen is Crt32.

16. A method for eliciting a cancer specific immune response, comprising: a) providing i) an immunogenic composition comprising a tumor antigen; and ii) a subject diagnosed with a cancer; and b) administering said immunogenic composition to said subject under conditions that said subject generates an immune response to said cancer.

17. The method of claim 16, wherein said tumor antigen is Crt32.

18. The method of claim 16, wherein said immunogenic composition further comprises an immune enhancing cytokine.

19. The method of claim 17, wherein said immune enhancing cytokine is expressed by a cell.

20. The method of claim 16, wherein said immune response results in a detectable decrease in the presence of said cancer.

21. The method of claim 16, wherein said immune response results in a measurable decrease in the level of said tumor antigen.

22. The method of claim 16, wherein said immune response results in a measurable decrease in the level of autoantibodies to said tumor antigen.

23. The method of claim 16, wherein said cancer is hepatocellular carcinoma.

24. The method of claim 16, wherein said subject is a human.

25. A method of treating cancer in a subject, comprising: a) providing i) a subject; and ii) a therapeutic composition comprising an antibody directed toward a tumor antigen; and b) administering said therapeutic composition to said subject.

26. The method of claim 25, wherein said tumor antigen is Crt32.

27. The method of claim 25, wherein said cancer is hepatocellular carcinoma.

28. The method of claim 25, wherein said antibody is attached to a cytotoxic agent.

29. The method of claim 28, wherein said cytotoxic agent is selected from the group consisting of chemotherapeutic agents, radioisotopes, cytosines, cytokines, and toxins.

30. The method of claim 28, wherein said cytotoxic agent is Ricin A chain.
