Claims
- 1. An endoscopic suturing instrument comprising:
a needle mounted at the distal end of the suturing instrument for the purpose of living-tissue puncture; an engaging means provided at least at the distal end of the needle for the purpose of connecting thread while allowing it to move freely; a needle-driving means to drive the needle; a loop-creating means to create a first loop in the thread, the first loop being formed by loosening the thread connected to said engaging means; a catching means to be inserted through said first loop to catch said thread disposed at the distal end of said suturing instrument body; and wherein the thread caught by said catching means can be moved freely and the needle-driving means and the catching means are integrated into one.
- 2. An endoscopic suturing instrument according to claim 1 wherein:
the loop-creating means is also used to create a second loop with the thread, the second loop being formed by loosening the thread between the living tissue and the catching means, the locus of the movement of the needle passes through the second loop.
- 3. An endoscopic suturing instrument according to claim 2 wherein:
the loop-creating means includes a cam mechanism in which the thread is released from the catching means when at least part of the needle passes through the second loop, after which the catching means is inserted through the first loop created by the loop-creating means so as to catch the thread.
- 4. An endoscopic suturing instrument according to claim 3 wherein:
the cam mechanism includes at least one spring member in order to direct the cam in a single direction.
- 5. An endoscopic suturing instrument according to claim 4 wherein:
the elastic member is a spring.
- 6. An endoscopic suturing instrument according to claim 1 wherein:
the loop-creating means creates at least one stitch.
- 7. An endoscopic suturing instrument according to claim 1 wherein:
the suturing instrument can be pulled out of a living body with the catching means holding the thread.
- 8. An endoscopic suturing instrument according to claim 1 wherein:
the catching means is in the shape of a hook so that it can catch the thread while allowing it to move freely.
- 9. An endoscopic suturing instrument according to claim 1 and further including:
means to prevent the thread from loosening so as to not allow the thread to become loose after suturing.
- 10. An endoscopic suturing instrument according to claim 1 and including a flexible endoscope.
- 11. An endoscopic suturing instrument according to claim 1 wherein:
the needle is a straight needle.
- 12. An endoscopic suturing instrument according to claim 1 wherein:
the needle is a curved needle.
- 13. An endoscopic suturing instrument according to claim 9 wherein:
the means to prevent the thread from loosening ties up both ends of the thread at least once.
- 14. An endoscopic suturing instrument according to claim 9 wherein:
the means to prevent the thread from loosening includes a thread-holding member on each end of the thread.
- 15. An endoscopic suturing instrument according to claim 14 wherein:
the thread-holding member is provided with a pair of arms to grasp thread.
- 16. An endoscopic suturing instrument according to claim 1 wherein:
the needle is provided with a needle-thrust-out supporting means on the side where the needle will thrust out of the elastic tissue so that the needle may thrust through the elastic tissue when the needle punctures the suture site with the thread connected to the engaging means.
- 17. An endoscopic suturing instrument according to claim 16 wherein:
the needle-thrust-out supporting means is allowed to move toward the side where the needle will thrust out of the tissue while not being allowed to move toward the opposite side.
- 18. An endoscopic suturing instrument according to claim 1 wherein:
the suturing instrument is provided with a fixing means with which to fix the living tissue and the suturing instrument temporarily.
- 19. An endoscopic suturing instrument according to claim 12 and further including:
a protecting member at its distal end of the endoscope or the suturing instrument with which to protect the living tissue from the curved needle when the endoscope is inserted into the living body.
- 20. A suturing method allowing continuous stitches using an endoscopic suturing instrument which includes:
a needle mounted at the distal end of the suturing instrument for the purpose of living-tissue puncture;an engaging means provided at least at the distal end of the needle for the purpose of connecting thread while allowing it to move freely; a needle-driving means to drive the needle; a loop-creating means to create a first loop in the thread, the first loop being formed by loosening the thread connected to said engaging means; a catching means to be inserted through said first loop to catch said thread disposed at the distal end of said suturing instrument body; and wherein the thread caught by said catching means can be moved freely and the needle-driving means and the catching means are integrated into one, comprising:
a. connecting the thread to the engaging means provided at a sharp tip of the needle, b. moving the suturing instrument to the first suture site of the living tissue, c. moving the needle to a first site where puncture is yet to be performed on the living tissue, d. moving the needle with the thread connected, to a second suture site where the needle thrusts out through the living tissue so that the thread comes out from the living tissue, e. moving the needle further along the puncture direction to the third site, f. returning the needle to the second site to create the first loop in the thread, g. catching the first loop with the catching means, h. moving the needle to the first the needle first position with the first loop caught by the catching means, i. moving the suturing instrument to the second suture site of the living tissue, j. creating a second loop between the living tissue and the catching means, k. moving the needle to the second site again, l. passing the needle through the second loop, m. removing the catching means from the thread forming the second loop when part of the needle passes through the second loop, n. moving the needle further along the puncture direction to a third site, o. returning the needle to the second site to create the first loop in the thread, p. catching the first loop with the catching means again, q. repeating steps h through p at least once, r. removing the suturing instrument from the lumen with the thread caught by the catching means, and s. stopping both ends of the thread that appear from the living tissue with the means to prevent the thread from loosening.
