Claims
- 1. An apparatus for reaming a medullary canal in a bone comprising:
- a flexible shaft having a distal end a rigid proximal end for engagement with a rotary power transmission source;
- a head having a distal end comprising a first reamer and a proximal end coupled to said distal end of said flexible shaft for rotation therewith;
- a second reamer having a bore defined therethrough for slideably receiving said flexible shaft, a proximal end, and a distal end;
- a drive coupling at least on said head and said second reamer such that at least said second reamer rotates with said first reamer and said flexible shaft; and
- a third reamer positioned on said flexible shaft to ream the proximal portion of a bone;
- wherein:
- when said second reamer is slid onto said flexible shaft, said distal end of said second reamer is adjacent said proximal end of said head and coupled directly by said drive coupling; and
- said flexible shaft is flexed from a straight configuration to position said third reamer to ream in an arc in a proximal portion of a bone.
- 2. The apparatus of claim 1, wherein:
- said third reamer has a bore defined therethrough for slideably receiving said flexible shaft;
- said third reamer has a distal end with a cavity shaped to engage said proximal end of said second reamer;
- a drive coupling is provided at said distal end of said third reamer and said proximal end of said second reamer; and
- said first, second and third reamers are coupled together via said drive couplings.
- 3. The apparatus of claim 2, wherein said second reamer has a generally conically shaped cutting portion tapering radially inwardly from said proximal end thereof to said distal end thereof.
- 4. The apparatus of claim 2, wherein said drive coupling at said distal end of said third reamer and said proximal end of said second reamer comprises pins and complementary holes mounted on contacting ends of a respective one of said second and third reamers.
- 5. The apparatus of claim 2, wherein said third reamer has a generally cylindrical cutting shape.
- 6. The apparatus of claim 1, wherein said flexible shaft has a stop element attached between said head and said second reamer to contact and locate said distal end of said second reamer with respect to the axial length of said flexible shaft.
- 7. The apparatus of claim 1, wherein said third reamer comprises:
- a concave cutting surface;
- a distal end having a cavity shaped to receive said rigid proximal end of said flexible shaft; and
- a proximal end having a drive connection shaft attached thereto, said drive connection shaft having a proximal end comprising a connector shaped to engage a rotary power transmission source.
- 8. The apparatus of claim 7, further comprising a template shaped to guide said third reamer in removing bone material from the calcar region of the long bone having a lateral elongated bore therein, said template comprising:
- an elongated member comprising a partially circular cavity having side edges and a rear wall partially comprising cutting teeth;
- an elliptical hollow member radiating from said side edges at an angle relative to the longitudinal axis of said elongated member, said hollow member having a rear wall comprising a portion of said rear wall of said elongated member;
- two protrusions extending distally from said hollow member and terminating in knife edges;
- impact receiving means extending laterally at the top edge of said elongated member and normal thereto; and
- anteversion control means extending laterally from, and normally to the longitudinal axis of, said elongated member.
- 9. The apparatus of claim 7, wherein:
- said third reamer is driven independently of said first and second reamers; and
- said cavity in said distal end of said third reamer is shaped to permit universal movement between said second and third reamers.
- 10. An apparatus for reaming the medial portion of a medullary canal in the calcar region of a long bone having a lateral bore, said reaming apparatus comprising:
- a flexible shaft having distal and proximal ends;
- a first element attached to said distal end of said flexible shaft and having an outer diameter about equal to the inner diameter of the lateral bore of the bone;
- a reamer having a distal end coupled to said proximal end of said flexible shaft, a proximal end, and a concave cutting surface for reaming a convex shape in the calcar portion of the bone; and
- a solid shaft having a distal end attached to said proximal end of said reamer and a proximal end; and
- a connector at said proximal end of said solid shaft shaped for engagement with a rotary power transmission source.
- 11. The apparatus of claim 10, wherein said first element comprises a smooth surface.
- 12. The apparatus of claim 10, wherein said first element comprises a cutting surface.
- 13. The apparatus of claim 10, further comprising a template shaped to guide said reamer in removing bone material from the calcar region of the long bone having a lateral elongated bore therein, said template comprising:
- an elongated member comprising a partially circular cavity having side edges and a rear wall partially comprising cutting teeth;
- an elliptical hollow member radiating from said side edges at an angle relative to the longitudinal axis of said elongated member said hollow member having a rear wall comprising a portion of said rear wall of said elongated member;
- two protrusions extending distally from said hollow member and terminating in knife edges;
- impact receiving means extending laterally at the top edge of said elongated member and normal thereto; and
- anteversion control means extending laterally from, and normally to the longitudinal axis of, said elongated member.
- 14. The apparatus of claim 10, further comprising a metaphyseal template comprising:
- an elongated member having a rear wall, side edges, and a distal end with a full, cannulated cylinder; and
- an elliptical hollow member radiating from said side edges at an angle relative to the longitudinal axis of said elongated member, said hollow member having a rear wall comprising a portion of said rear wall of said elongated member;
- wherein the outer diameter of said cannulated cylinder is about equal to the inner diameter of the lateral bore of the bone at the isthmus and the outer diameter of element of said reaming apparatus is about equal to the inner diameter of said cannulated cylinder.
- 15. An apparatus for reaming the medial portion medullary canal in the calcar region of a long bone, containing a lateral bore, said reaming apparatus comprising:
- a metaphyseal template having an elongated member with a distal end, a cannulated cylinder at said distal end of said elongated member, and a hollow member radiating from said elongated member;
- a flexible shaft having a distal end and a proximal end;
- a first element attached to said distal end of said flexible shaft and having an outer diameter equal to the inner diameter of said cannulated cylinder of said template;
- a reamer having a proximal end, a distal end fixedly attached to said proximal end of said flexible shaft, and a cylindrical cutting surface for reaming a convex shape in the calcar portion of the bone; and
- a rigid shaft having a distal end fixedly attached to said proximal end of said reamer and a proximal end having a connector shaped for engagement of said rigid shaft with a rotary power transmission source; and
- a fluted portion attached to and spaced between said distal and proximal ends of said rigid shaft to guide said flexible shaft within said hollow portion of said template.
- 16. A method of preparing a long bone, comprising an opening in the trochanteric fossa, to receive a prosthesis, said method comprising the steps of:
- initially forming a lateral bore by means of a reaming apparatus having at least a proximal reamer and a distal reamer coupled to a flexible shaft; and then
- reaming the calcar portion of the bone to complete the shape of the prosthesis by flexing said flexible shaft of said reaming apparatus to position said proximal reamer against the calcar position.
- 17. The method of claim 16, wherein said bone comprises a bore to receive a guide wire and said reaming apparatus is cannulated to pass the guide wire therethrough.
- 18. The method of claim 16, wherein:
- said proximal reamer is replaced by a third reamer having a proximal end with a connector coupled thereto, a distal end with a cavity defined therein, and a concave cutting surface;
- said proximal portion of said flexible shaft has a connector inserted into said cavity of of said third reamer;
- said connector on said third reamer is coupled with the rotary power transmission source; and
- said proximal end of said flexible shaft is flexed and the calcar bone area reamed by said third reamer to complete the shape of the prosthesis.
- 19. The method of claim 16, further comprising the steps of:
- removing said reaming apparatus;
- inserting the head of a second reaming apparatus having a reamer with a concave cutting surface with a distal edge into the bore until the distal edge of said cutting surface of said reamer reaches the proximal portion of the calcar region;
- removing the calcar bone in a lateral-medial direction until the cortical bone is reached;
- further inserting said head into the bore and removing additional calcar bone until the cortical bone is again reached; and
- repeating the above process until all of the bone in the calcar region has been removed.
- 20. The method of claim 18, wherein a metaphyseal template is inserted into the lateral bore of the bone prior to formation of the calcar region and said proximal reamer is replaced by said third reamer.
- 21. A method of preparing a bone to receive a prosthesis, said method comprising the steps of:
- inserting into the lateral bore of the bone a metaphyseal template comprising an elongated member having a partially circular cavity with a rear wall partially comprising cutting teeth, a cannulated cylinder at a distal end of said elongated member, an elliptical hollow member radiating from said elongated member at an angle to the longitudinal axis of the elongated member, impact receiving means extending laterally at the top of said elongated member and normal thereto, and anteversion control means extending laterally from and normal said elongated member;
- seating said template at the proximal end of the calcar portion of the bone by means of the impact receiving means and the anteversion control means;
- providing a reamer apparatus comprising a first head at a distal end of said reamer apparatus, a flexible shaft, and a reamer at a proximal end of said flexible shaft;
- passing said first head of said reamer apparatus through said hollow portion of said template and into said cannulated cylinder; and
- reaming the calcar region of the bone by flexing said flexible shaft of said reamer apparatus and manipulating said reamer in a medial direction in said hollow portions of said hollow member by a series of motions.
- 22. An apparatus for reaming a bone canal comprising:
- a flexible shaft having a first end shaped for engagement with a rotary power transmission source, and a second end;
- a first reamer coupled to said second end of said flexible shaft for rotation therewith;
- a second reamer having a bore therethrough for slideably receiving said flexible shaft, said second reamer having a first end which, when said second reamer is slid onto said flexible shaft, is adjacent said first reamer, and a second end; and
- a third reamer having a bore therethrough for slideably receiving said flexible shaft, said third reamer having a first end for engaging a second end of said second reamer shaped to permit universal movement between said second and third reamers
- wherein:
- said first and second reamers have a drive coupling therebetween such that said first and second reamers are rotated together with said flexible shaft;
- said third reamer is driven independently from said first and second reamers; and
- said flexible shaft is flexed from a straight configuration to position said third reamer to ream in an arc in a proximal portion of a bone.
- 23. The apparatus of claim 22, wherein said second reamer has a generally conically shaped cutting portion tapering radially inwardly from said second end thereof to said first end thereof.
- 24. The apparatus of claim 22, wherein said third reamer has a concave cutting surface for cutting a convex shape in bone surrounding the bone canal.
- 25. The apparatus of claim 22, wherein said flexible shaft has a stop element attached thereto to contact and locate said first end of said second reamer with respect to the axial length of said flexible shaft.
- 26. The apparatus as set forth in claim 22, wherein said first end of said third reamer and said second end of said second reamer as a ball and socket joint with one of said ball and socket mounted on contacting ends of a respective one of said second and third reamers.
- 27. The apparatus as set for the in claim 22, wherein said third reamer has a generally cylindrical cutting surface.
- 28. An apparatus for reaming a femoral medullary canal comprising:
- a flexible drive shaft having a proximal end with a solid shaft portion and a distal portion;
- a first reamer coupled to said distal portion of said flexible shaft and driven by said flexible shaft;
- a second reamer having a concave cutting surface, said second reamer mounted on a proximal portion of said flexible shaft for reaming a convex shape on the proximal portion of said femoral canal; and
- means for rotating said first and second reaming elements.
- 29. The apparatus as set forth in claim 28, further including a third reamer intermediate said first and second reamers, said third reamer having a bore therethrough, a distal end adjacent said first reamer, and a proximal end adjacent said second reamer for reaming a midshaft region of said canal.
- 30. The apparatus as set forth in claim 28, wherein said second reamer has a bore therein for slideably receiving said flexible shaft.
- 31. The apparatus as set forth in claim 30, wherein said means for rotating said second reamer is independent of said flexible shaft.
