Claims
- 1. An implantable pulse generation means for delivering an electrical energy pulse to the heart comprising:
- a pulse generator for generating the electrical energy pulse;
- a first cardiac electrode connected to the pulse generator for implantation in the region of the heart to deliver the electrical energy pulse thereto;
- a second cardiac electrode connected to the pulse generator for implantation in the region of the heart to deliver the electrical energy pulse thereto;
- a housing body enclosing and containing the pulse generator having an electrically conductive outer surface electrically connected to said pulse generator as a second cardiac electrode; and
- means for directing the electrical energy pulse between the first and second electrodes upon a first condition of the heart, and directing said electrical energy pulse to the first electrode, and the conductive surface of the pulse generator housing electrically connected in common with the first electrode, against said second electrode upon a second condition of the heart.
- 2. The implantable pulse generation means of claim 1, wherein the outer surface of said housing body comprises platinum.
- 3. The implantable pulse generation means of claim 1, wherein the outer surface is microporous.
- 4. The implantable pulse generation means of claim 1, said outer surface of the housing body including a plurality of regions which are electrically isolated, select regions of said plurality of regions being dedicated for sensing cardiac signals and other regions of said plurality of regions being dedicated for discharging to the heart.
- 5. The implantable pulse generation means of claim 4, wherein the select regions further include a ground electrode, a pacing electrode and a sensing electrode.
- 6. The implantable pulse generation means of claim 5, wherein a select region which serves as pacing or sensing electrode comprises insulative material deposited on said housing body over a predetermined area and electrically conductive material deposited on the insulative material, said electrically conductive material occupying a fraction of said predetermined area.
- 7. The implantable pulse generation means of claim 1, further comprising an insulative mask disposed over the outer surface of the housing body.
- 8. The implantable pulse generation means of claim 7, further including a sensing electrode implanted in the region of the heart and connected to said switching means for sensing at least said first and second conditions of the heart.
- 9. The implantable pulse generation means of claim 1, further comprising a second electrically conductive surface being joined to the pulse generator housing and being electrically connected to the electrically conductive outer surface such that the second electrically conductive surface and the electrically conductive outer surface together serve as an electrode.
- 10. The implantable pulse generation means of claim 9, further comprising a support layer adjoining the pulse generator housing and disposed between the pulse generator housing and the electrically conductive outer surface for supporting and positioning the electrically conductive outer surface, and a lead for electrically connecting the electrically conductive outer surface to the pulse generator.
- 11. The implantable pulse generation means of claim 10, wherein the second electrically conductive surface is a plurality of coiled segment electrodes protruding from the support layer.
- 12. The implantable pulse generation means of claim 9, wherein the second electrically conductive surface is a plurality of coiled segment electrodes protruding from the pulse generator housing.
- 13. The implantable pulse generation means of claim 9, wherein the second electrically conductive surface is a coiled loop.
- 14. The implantable pulse generation means according to claim 1, wherein said at least 1 electrode includes means for implantation directly on the heart.
- 15. The implantable pulse generation means according to claim 1, said housing body having a generally two dimensional electrically conductive side, the conductive side including means for directing electrical energy toward the heart.
- 16. An implantable heart treatment system having anti-arrhythmia pacemaking, cardioversion, and defibrillation capabilities for maintaining proper function of the heart, said system comprising:
- a first cardiac electrode connected to the pulse generating for implantation in the region of the heart to deliver the electrical energy pulse thereto;
- a second cardiac electrode connected to the pulse generator for implantation in the region of the heart to deliver the electrical energy pulse thereto;
- an implantable pulse generator means selectively producing an anti-arrhythmia waveform for arrhythmia pacemaking, cardioversion, or defibrillation;
- a pulse generator housing for implantation proximate to the heart enclosing and containing said pulse generator means, said pulse generator housing formed of electrically conductive material defining an electrically conductive outer surface which is electrically connected to said pulse generator means; and
- switching means for directing on the one hand, said anti-arrhythmia waveform from said first electrode to the pulse generator housing upon a first condition of the heart, and on the other hand directing said anti-arrhythmia waveform from said first and second electrodes against said conductive outer surface of said pulse generator housing.
- 17. The implantable pulse generator housing of claim 16, further comprising a second electrically conductive surface adjoining the pulse generator housing and being electrically connected to the electrically conductive outer surface such that the second electrically conductive surface and the electrically conductive outer surface together serve as an electrode.
- 18. An implantable heart treatment system having anti-arrhythmia pacemaking, cardioversion, and defibrillation capabilities for maintaining proper function of the heart, said system comprising:
- an implantable pulse generator means selectively producing an anti-arrhythmia waveform for arrhythmia pacemaking, cardioversion, and defibrillation;
- a pulse generator housing enclosing and containing said pulse generator means, said pulse generator housing formed of electrically conductive material defining an electrically conductive outer surface electrically connected to said pulse generator means for delivering electrical energy to the heart, said housing for implantation proximate the heart;
- a first electrode for implantation in the human body and being electrically connected to said pulse generator;
- a second electrode for implantation in the human body and being electrically connected to said pulse generator;
- switching means for directing said anti-arrhythmia waveform on the one hand, between said first and second electrodes upon a first condition of said heart, and on the other hand directing said anti-arrhythmia waveform to said first electrode and the conductive surface of said pulse generator housing electrically connected in common with the first electrode against said second electrode upon a second condition of the heart.
- 19. The implantable heart treatment system of claim 18, wherein at least one of said first and second electrodes has heart condition sensing capabilities and is connected to said switching means for sensing at least said first and second conditions of the heart.
- 20. The implantable heart treatment system of claim 18, wherein said conductive surface of said pulse generator housing comprises electrically conductive mesh.
- 21. The implantable heart treatment system of claim 20, and further including a third electrode implanted in the human body and electrically connected to said switching means for sensing at least said first and second conditions of the heart.
- 22. The implantable heart treatment system of claim 20, wherein at least one of said first and second electrodes has sensing capabilities and is connected to said switching means for sensing at least said first and second conditions of the heart.
- 23. The implantable heart treatment system of claim 20, wherein said second condition of the heart is ventricular fibrillation.
- 24. The implantable heart treatment system of claim 18, and further comprising sensor means connected to said switching means, said sensor means for detecting whether the pulse generator housing is inside a body of a patient or outside a body of a patient and responsive to control the switching means to disconnect the electrically conductive outer surface from the pulse generator means when the sensor means detects that the housing is outside the body of the patient.
- 25. An implantable heart treatment system having anti-arrhythmia pacemaking, cardioversion, and defibrillation capabilities for maintaining proper function of the heart, said system comprising:
- an implantable pulse generator selectively producing an anti-arrhythmia waveform for arrhythmia pacemaking, cardioversion, and defibrillation;
- a pulse generator housing for enclosing and containing said pulse generator, said pulse generator housing formed of electrically conductive material defining an electrically conductive outer surface which is electrically connected to said pulse generator;
- a transvenous lead having a first electrode and a second electrode portion thereof, said lead for implantation in the heart with said first electrode in the right ventricle and said second electrode is positioned in the vena cava region of the heart, said lead electrically connecting said first and second electrode to said pulse generator;
- switching means for directing on the one hand, said anti-arrhythmia waveform between said first and second electrodes upon a first condition of the heart, and on the other hand directing said anti-arrhythmia waveform to said first electrode and said conductive outer surface of said pulse generator housing electrically connected in common with said first electrode against said second electrode upon a second condition of the heart.
- 26. The implantable heart treatment system of claim 25, further comprising a second electrically conductive surface adjoining the pulse generator housing and being electrically connected to the electrically conductive outer surface, the electrically conductive outer surface and the second electrically conductive surface together serving as an electrode.
- 27. An implantable pulse generator housing for enclosing and containing a pulse generator for connection to a first and a second electrode implanted in the region of the heart for delivering electrical energy to the heart, said housing comprising:
- a plurality of walls, at least one of which comprises an electrically conductive electrode connected to said pulse generator as a third cardiac electrode and removably attached to at least one wall; and
- means for directing the electrical energy pulse between the first and second electrodes upon a first condition of the heart, and directing said electrical energy pulse to the first electrode and the third electrode, against said second electrode upon a second condition of the heart.
- 28. An implantable pulse generation means comprising:
- a pulse generator;
- a first cardiac electrode connected to the pulse generator and implanted in close proximity to the heart;
- a second cardiac electrode connected to the pulse generator and implanted in close proximity to the heart; and
- a generator housing enclosing and containing the pulse generator having at least four side walls, a top wall, and a bottom wall, at least one of said walls being electrically conductive and electrically connected to said pulse generator as a third cardiac electrode; and
- means for directing the electrical energy pulse between the first and second electrodes upon a first condition of the heart, and directing said electrical energy pulse from the first, second, and third electrode upon a second condition of the heart.
- 29. The implantable pulse generator housing of claim 28, further comprising a second electrically conductive surface adjoining the generator housing and being electrically connected to the at least one electrically conductive wall such that the second electrically conductive surface and the electrically conductive outer surface together serve as the third electrode.
- 30. The implantable pulse generation means according to claim 28, wherein said at least 1 electrode includes means for implantation directly on the heart.
