Claims
- 1. An apparatus for patient treatment comprising:
- an outer catheter having a proximal end and a distal end having at least a first passage extending lengthwise therein;
- an inner catheter disposed at least partly in said first passage and having a proximal end and a distal end, said inner catheter having an extending portion extendable out said distal end of said outer catheter by a variable amount;
- lengthwise offset first and second electrodes, said first electrode mounted on an external surface of said side wall and said second electrode mounted on a surface selected from the group consisting of: the external surface of said side wall and an external surface of said outer catheter, said first and second electrodes operable for detecting the presence of conductive cardiac tissue; and
- a steering means for steering said outer catheter.
- 2. The apparatus of claim 1 wherein said inner catheter is rotatable relative to said outer catheter.
- 3. The apparatus of claim 2 wherein said second electrode is on said external surface of said side wall.
- 4. The apparatus of claim 3 further comprising a third electrode on said external surface of said outer catheter.
- 5. The apparatus of claim 2 wherein said second electrode is on said external surface of said outer catheter such that extension of said inner catheter relative to said outer catheter varies the distance between said first and second electrodes.
- 6. The apparatus of claim 2 wherein said first and second electrodes have contact surfaces which are limited to an angular range excluding at least a back part of said side wall radially opposite said first window.
- 7. The apparatus of claim 6 wherein said first and second electrodes are respectively first and second rings and a back part of each ring is covered by insulating material.
- 8. The apparatus of claim 2 further comprising a third electrode, and wherein said second and third electrodes are mounted on said external surface of said side wall for detecting the presence of conductive cardiac tissue.
- 9. The apparatus of claim 8 further comprising a fourth electrode mounted on said external surface of said side wall for detecting the presence of conductive cardiac tissue.
- 10. The apparatus of claim 1 wherein the inner and outer catheters respectively are inner and outer parts of a single catheter and the inner part is captured to the outer part.
- 11. The apparatus of claim 1 further including a non-optical ablation device to provide ablation energy near to the first and second electrodes.
- 12. The apparatus of claim 11 wherein said non-optical ablation device includes an radio frequency source.
- 13. A method of treating a patient comprising the steps of:
- inserting a catheter at least partly within a patient, the catheter having:
- an outer catheter having a proximal end and a distal end having at least a first passage extending lengthwise therein;
- an inner catheter disposed at least partly in said first passage and having a proximal end and a distal end, said inner catheter having an extending portion extendable out said distal end of said outer catheter by a variable amount;
- lengthwise offset first and second electrodes, said first electrode mounted on an external surface of said side wall and said second electrode mounted on a surface selected from the group consisting of: the external surface of said side wall and an external surface of said outer catheter, said first and second electrodes operable for detecting the presence of conductive cardiac tissue; and
- a steering means for steering said outer catheter; and wherein said steering means includes a coil spring mounted near said distal end of said outer catheter and a control means for bending said coil spring by manipulation at a proximal end of the steering means;
- using said steering means to position a distal end of said catheter at a desired location within a patient's heart such that said spring means is bent so as to push a distal portion of the catheter against a wall of the patient's heart.
- 14. The method of claim 13 wherein said spring means is bent so as to push a distal portion of the catheter against a wall of the patient's heart and thus maintain said first and second electrodes in contact with the wall of the patient's heart, even during all parts of the heart beat; and further including the step of mapping portions of the patient's heart using the first and second electrodes.
- 15. A method of treating a patient comprising the steps of:
- inserting a catheter at least partly within a patient, the catheter having:
- an outer catheter having a proximal end and a distal end having at least a first passage extending lengthwise therein;
- an inner catheter disposed at least partly in said first passage and having a proximal end and a distal end, said inner catheter having an extending portion extendable out said distal end of said outer catheter by a variable amount; and a steering means for steering said outer catheter; and wherein said steering means includes a coil spring mounted near said distal end of said outer catheter and a control means for bending said outer catheter by bending the coil spring by manipulation at a proximal end of the steering means;
- using said steering means to position a distal end of said catheter at a desired location within a patient's heart such that said spring means is bent so as to push a distal portion of the catheter against a wall of the patient's heart.
- 16. The method of claim 15 wherein said inner catheter includes first and second electrodes operable for detecting the presence of cardiac conductive tissue and wherein said spring means is bent so as to push a distal portion of the catheter against a wall of the patient's heart and thus maintain said first and second electrodes in contact with the wall of the patient's heart, even during all parts of the heart beat.

This application is a continuation-in-part of U.S. patent application Ser. No. 08/123,207 filed Sep. 20, 1993, now U.S. Pat. No. 5,464,404 in the names of Abela, Friedl, and Mathews, the inventors herein. A divisional application Ser. No. 08/394,763 of that application was filed Feb. 27, 1995, now U.S. Pat. No. 5,575,787. Another continuation-in-part of U.S. patent application Ser. No. 08/123,207 is being filed concurrently herewith in the names of Abela, Friedl, and Mathews and titled OPTICAL FIBER CATHETER AND METHOD. Another continuation-in-part of U.S. patent application Ser. No. 08/123,207 is being filed concurrently herewith in the names of Abela, Friedl, Mathews and Bowden and titled CARDIAC CATHETER ANCHORING AND METHOD. All of the referenced applications including their claims, drawings and text as originally filed and all amendments made prior to the present filing date are hereby incorporated by reference.
