The present disclosure pertains to medical devices, and methods for manufacturing medical devices. More particularly, the present disclosure pertains to medical devices with a removable liner.
A wide variety of intracorporeal medical devices have been developed for medical use, for example, intravascular use. Some of these devices include guidewires, catheters, and the like. These devices are manufactured by any one of a variety of different manufacturing methods and may be used according to any one of a variety of methods. Of the known medical devices and methods, each has certain advantages and disadvantages. There is an ongoing need to provide alternative medical devices as well as alternative methods for manufacturing and using medical devices.
This disclosure provides design, material, manufacturing method, and use alternatives for medical devices and/or medical device systems. An example medical device system is disclosed. The system comprises:
a catheter shaft having a proximal end;
a removable liner extending through the catheter shaft;
wherein the removable liner is designed to deliver a medical substance to a target region;
wherein the removable liner is designed to be removed from the catheter shaft if the removable liner becomes plugged;
a replacement liner;
wherein the replacement liner is designed to be inserted within the catheter shaft after the removal of the removable liner; and
a support member disposed along the replacement liner.
Alternatively or additionally to any of the embodiments above, the catheter shaft includes a first hub and wherein the removable liner includes a second hub designed to engage the first hub.
Alternatively or additionally to any of the embodiments above, the first hub, the second hub, or both include a threaded connector.
Alternatively or additionally to any of the embodiments above, the catheter shaft includes a reinforcing member.
Alternatively or additionally to any of the embodiments above, the removable liner includes a reinforcing member.
Alternatively or additionally to any of the embodiments above, the replacement liner defines a lumen and wherein the support member includes a delivery stylet disposed within the lumen.
Alternatively or additionally to any of the embodiments above, the support member includes a breakaway sheath disposed along an outer surface of the replacement liner.
Alternatively or additionally to any of the embodiments above, the removable liner, the replacement liner, or both are designed to shift between a collapsed configuration and a non-collapsed configuration.
Alternatively or additionally to any of the embodiments above, the removable liner, the replacement liner, or both include a temporary distal seal member.
An example medical device is disclosed. The medical device comprises:
a tubular member having a proximal end region and a distal end region;
a first hub member coupled to the proximal end region;
a removable liner extending through the tubular member;
wherein the removable liner is free from attachment with the tubular member;
a second hub member coupled to the removable liner;
wherein the first hub member and the second hub member are designed to be detachably nested with one another; and
wherein the removable liner is designed to be removed from the tubular member so that a second liner may be disposed within the tubular member.
Alternatively or additionally to any of the embodiments above, the tubular member includes a reinforcing member.
Alternatively or additionally to any of the embodiments above, the removable liner includes a reinforcing member.

An example method for treating a body lumen is disclosed. The method comprises:
positioning an embolic material delivery device adjacent to a target region;
wherein the embolic material delivery device comprises:
advancing a first medical substance through the removable liner to a position adjacent to the target region;
removing the removable liner from the catheter shaft;
advancing a replacement liner through the catheter shaft; and
advancing a second medical substance through the replacement liner, wherein the second medical substance is the same as or different from the first medical substance.
Alternatively or additionally to any of the embodiments above, the replacement liner is prefilled with the second medical substance.
Alternatively or additionally to any of the embodiments above, the method may include advancing a third medical substance through the catheter shaft.

Medical devices including catheters and/or microcatheters may be used to diagnose and/or treat a number of different conditions. For example, in certain scenarios it may be desirable to utilize a microcatheter to perform an embolization procedure. Such a procedure may include the infusion of an embolic material (e.g., an embolic glue, gel, foam, or the like) into a body lumen in order to seal off or otherwise block the body lumen (e.g., block a blood vessel, fill an aneurism, etc.). In some instances, the embolic material itself may be tacky, sticky, or clumpy. This may aid in the function of the embolic material. However, when such materials are used, the microcatheter used to deliver the materials could become blocked or otherwise obstructed. When this happens, the microcatheter may be removed from the body lumen and replaced with a new device. While replacing the medical device can be carried out by clinician, such a procedure may be time consuming and/or technically challenging. Disclosed herein are medical devices and/or medical device systems that allow a plugged or clogged microcatheter to be efficiently “unplugged” without having to replace the entire device. Some additional details of the medical devices/systems are disclosed herein.
In general, medical substance 14 may be delivered through catheter shaft 16 by passing medical substance 14 through liner 18. For example, in some instances, a medical substance delivery member 29a such as a syringe may be coupled to liner 18 (e.g., at second hub member 22) as shown schematically in
As described herein, in some instances medical substance 14 may have a tacky or sticky consistency. Because of this, medical substance 14 could become stuck or clogged within, for example, liner 18. This may include medical substance 14 sticking to the outer surface of liner 18 (e.g., in instances where liner 18 extends distally beyond the distal end of catheter shaft 16), sticking to the inner surface of liner 18, clumping or clogging the lumen 28 of liner 18, or the like. In
Liner 18 of system 10 may be designed to be “removable”. In other words, liner 18 may be removed from catheter shaft 16 so that it can be replaced with a second or “replacement” liner, for example when liner 18 is clogged with medical substance 14. In addition, in some situations it may be desirable to stop or limit infusions of medial substance 14 after some time or after a suitable quantity of medical substance 14 has been delivered. This may allow for a different medical substance 14 to be delivered, if desired. Therefore, in such situations it may be desirable to remove liner 18, even though liner 18 may not necessarily be clogged.
In order for liner 18 to be a replaceable liner 18, it may be desirable for liner 18 to be free from attachment with catheter shaft 16. For example, along the length of liner 18, liner 18 may be free from being bonded to, glued to, or otherwise secured with catheter shaft 16 (e.g., the inner surface of catheter shaft 16). In some instances, the only location where liner 18 and catheter shaft 16 are interconnected may be at hub members 20/22 (e.g., where hub member 20/22 are detachably connected). However, in other instances, other portions of liner 18 and catheter shaft 16 may be detachably connected including along the respective lengths thereof.
After the replaceable liner 18 has been removed from catheter shaft 16, a replacement liner 18′ may be advanced through catheter shaft 16 as shown in
In at least some instances, support member 32 may take the form of a delivery stylet 32a disposed within the lumen 28′ of replacement liner 18′. Stylet 32a may take the form of a shaft or wire that can be disposed within replacement liner 18′. In some instances, stylet 32a may simply sit within replacement liner 18′ during advancement of replacement liner 18′ through catheter shaft 16. In other instances, stylet 32a may engage a ridge, shoulder, radially inward projection, narrowed region, or the like within replacement liner 18′ so that stylet 32a can “push” replacement liner 18′ distally within catheter shaft 16. Other arrangements are contemplated. Stylet 32a may be a solid (e.g., non-tubular) structure or a tubular structure. In some instances, stylet 32a may be or resemble a guidewire.

In at least some instances, replacement liner 18′ may be prefilled with medical substance 14 and/or a different medical substance. For example, liner 18 may be used to deliver medical substance 14. Liner 18 may be removed and prefilled replacement liner 18′ may be disposed within catheter shaft 16 so that another medical substance may be delivered. Prefilled replacement liner 18′ may have sufficient strength so that prefilled replacement liner 18′ may be disposed within catheter shaft 16 without the need of support member 32. However, in other instances, support member 32 (e.g., support member 32b) may be utilized with prefilled replacement liner 18′.

The materials that can be used for the various components of system 10 (and/or other systems disclosed herein) and the various tubular members disclosed herein may include those commonly associated with medical devices. For simplicity purposes, the following discussion makes reference to liner 18 and other components of system 10. However, this is not intended to limit the devices and methods described herein, as the discussion may be applied to other structures and components of the systems disclosed herein.
Liner 18 and/or other components of system 10 may be made from a metal, metal alloy, polymer (some examples of which are disclosed below), a metal-polymer composite, ceramics, combinations thereof, and the like, or other suitable material. As alluded to herein, within the family of commercially available nickel-titanium or nitinol alloys, is a category designated “linear elastic” or “non-super-elastic” which, although may be similar in chemistry to conventional shape memory and super elastic varieties, may exhibit distinct and useful mechanical properties. Linear elastic and/or non-super-elastic nitinol may be distinguished from super elastic nitinol in that the linear elastic and/or non-super-elastic nitinol does not display a substantial “superelastic plateau” or “flag region” in its stress/strain curve like super elastic nitinol does. Instead, in the linear elastic and/or non-super-elastic nitinol, as recoverable strain increases, the stress continues to increase in a substantially linear, or a somewhat, but not necessarily entirely linear relationship until plastic deformation begins or at least in a relationship that is more linear that the super elastic plateau and/or flag region that may be seen with super elastic nitinol. Thus, for the purposes of this disclosure linear elastic and/or non-super-elastic nitinol may also be termed “substantially” linear elastic and/or non-super-elastic nitinol.
In some cases, linear elastic and/or non-super-elastic nitinol may also be distinguishable from super elastic nitinol in that linear elastic and/or non-super-elastic nitinol may accept up to about 2-5% strain while remaining substantially elastic (e.g., before plastically deforming) whereas super elastic nitinol may accept up to about 8% strain before plastically deforming. Both of these materials can be distinguished from other linear elastic materials such as stainless steel (that can also can be distinguished based on its composition), which may accept only about 0.2 to 0.44 percent strain before plastically deforming.
In some embodiments, the linear elastic and/or non-super-elastic nickel-titanium alloy is an alloy that does not show any martensite/austenite phase changes that are detectable by differential scanning calorimetry (DSC) and dynamic metal thermal analysis (DMTA) analysis over a large temperature range. For example, in some embodiments, there may be no martensite/austenite phase changes detectable by DSC and DMTA analysis in the range of about −60 degrees Celsius (° C.) to about 120° C. in the linear elastic and/or non-super-elastic nickel-titanium alloy. The mechanical bending properties of such material may therefore be generally inert to the effect of temperature over this very broad range of temperature. In some embodiments, the mechanical bending properties of the linear elastic and/or non-super-elastic nickel-titanium alloy at ambient or room temperature are substantially the same as the mechanical properties at body temperature, for example, in that they do not display a super-elastic plateau and/or flag region. In other words, across a broad temperature range, the linear elastic and/or non-super-elastic nickel-titanium alloy maintains its linear elastic and/or non-super-elastic characteristics and/or properties.
In some embodiments, the linear elastic and/or non-super-elastic nickel-titanium alloy may be in the range of about 50 to about 60 weight percent nickel, with the remainder being essentially titanium. In some embodiments, the composition is in the range of about 54 to about 57 weight percent nickel. One example of a suitable nickel-titanium alloy is FHP-NT alloy commercially available from Furukawa Techno Material Co. of Kanagawa, Japan. Some examples of nickel titanium alloys are disclosed in U.S. Pat. Nos. 5,238,004 and 6,508,803, which are incorporated herein by reference. Other suitable materials may include ULTANIUM™ (available from Neo-Metrics) and GUM METAL™ (available from Toyota). In some other embodiments, a superelastic alloy, for example a superelastic nitinol can be used to achieve desired properties.
In at least some embodiments, portions or all of liner 18 and/or other components of system 10 may also be doped with, made of, or otherwise include a radiopaque material. Radiopaque materials are understood to be materials capable of producing a relatively bright image on a fluoroscopy screen or another imaging technique during a medical procedure. This relatively bright image aids the user of system 10 in determining its location. Some examples of radiopaque materials can include, but are not limited to, gold, platinum, palladium, tantalum, tungsten alloy, polymer material loaded with a radiopaque filler, and the like. Additionally, other radiopaque marker bands and/or coils may also be incorporated into the design of system 10 to achieve the same result.
In some embodiments, a degree of Magnetic Resonance Imaging (MRI) compatibility is imparted into system 10. For example, liner 18 and/or other components of system 10 may be made of a material that does not substantially distort the image and create substantial artifacts (e.g., gaps in the image). Certain ferromagnetic materials, for example, may not be suitable because they may create artifacts in an MM image. Liner 18 and/or other components of system 10 may also be made from a material that the MRI machine can image. Some materials that exhibit these characteristics include, for example, tungsten, cobalt-chromium-molybdenum alloys (e.g., UNS: R30003 such as ELGILOY®, PHYNOX®, and the like), nickel-cobalt-chromium-molybdenum alloys (e.g., UNS: R30035 such as MP35-N® and the like), nitinol, and the like, and others.
It should be understood that this disclosure is, in many respects, only illustrative. Changes may be made in details, particularly in matters of shape, size, and arrangement of steps without exceeding the scope of the disclosure. This may include, to the extent that it is appropriate, the use of any of the features of one example embodiment being used in other embodiments.
