The embodiments described herein relate generally to the field of ophthalmic therapies and more particularly to the use of an extraction system for extracting medicaments or any other fluid from a container, for example, a vial, for subsequent ocular injection for treatment of an eye.
When transferring medicament from a medicament container (e.g., a vial) to a delivery device, in many instances it is desirable to minimize the amount of medicament that is not accessible from the medicament container (e.g., minimize dead volume), and thereby minimize any amount of wasted medicament. This can be particularly important, for example, in situations in which a procedure includes multiple injection attempts, inevitably leading to at least some wasted medicament. For example, in some instances a delivery device is configured to work with several different sized needles, e.g., to accommodate for a patient's particular anatomy. An operator may first attempt to inject a medicament into a patient's eye using a puncture member (e.g., a microneedle) having a first length, but fail to suitably reach the targeted injection region (e.g., the SCS or SRS). Accordingly, in such instances, the operator will swap the puncture member having the first length with another puncture member having a second length more suitable for that particular patient's anatomy. Often the first attempt will result in some wasted medicament, and therefore additional medicament will need to be loaded into the delivery device prior to the second attempt. In this scenario, it can be very important to maximize access to the medicament within the medicament container to ensure enough medicament is available for the second attempt.
Additionally, some medicament containers are designed such that a portion of the medicament remains unused. This can lead to medicament being wasted. Designing a medicament container for ease of outflow and access can maximize the efficiency of medicament use.
Further, in some situations a delivery device may need to extract medicament from a single medicament container multiple times, for example, in situations in which an operator needs to swap delivery device puncture members. In such situations, the medicament within the medicament container may be exposed to the environment (and thus susceptible to contamination or degradation), such that the medicament is less suitable for use.
Thus, a need exists for improved extraction devices and methods for transferring medicament from a medicament container into a delivery device.
Embodiments described herein relate to methods and apparatus for extracting a fluid from a container. In some aspects, an extraction member can comprise a first coupling portion configured to removably couple the extraction member to a medicament container containing a medicament such that the first coupling portion at least partially circumferentially surrounds a septum seal of the medicament container, a second coupling portion configured to removably couple the extraction member to a medicament injector, a puncture member defining a fluidic channel and configured to pierce the septum seal to establish fluidic communication between the medicament container and the medicament injector via the fluidic channel as the extraction member is coupled to the medicament container via the first coupling portion, and a seal disposed between the first coupling portion and the second coupling portion, and defining an opening through which the puncture member is disposed and extends therefrom towards the first coupling portion thereby defining a medicament collection area circumferentially surrounding a portion of the puncture member between the seal and a distal end of the puncture member, the seal configured to abut the septum seal when the first coupling portion is removably coupled to the medicament container such that any medicament extracted from the medicament container is limited to the collection area and the fluidic channel. The puncture member includes an aperture configured to fluidically couple the fluidic channel of the puncture member with the collection area at a low point of the collection area during extraction of medicament from the medicament container to the medicament injector through the fluidic channel to maximize transfer of any medicament from the collection area to the fluidic channel.
In some embodiments, the aperture extends to a point less than 50 um from a bottom of the extraction member. In some embodiments, the puncture member includes at least one of a hard plastic or a metal.
In some embodiments, the first coupling portion includes a plurality of arms configured to flex open to secure the extraction member around a cap of the medicament container.
In some embodiments, each of the arms includes a ledge configured to secure the cap of the medicament container.
In some embodiments, the second coupling portion includes a plurality of ridges configured to facilitate gripping the second coupling portion.

In some embodiments, the method further comprises removing the medicament injector from the extraction member and securing a sealing member to the second coupling portion to block the flow of fluid through the aperture.

As used herein, the term “suprachoroidal space,” or SCS which is synonymous with suprachoroid, or suprachoroidia, describes the space (or volume) and/or potential space (or potential volume) in the region of the eye 10 disposed between the sclera 20 and choroid 28. This region primarily is composed of closely packed layers of long pigmented processes derived from each of the two adjacent tissues; however, a space can develop in this region because of fluid or other material buildup in the suprachoroidal space and the adjacent tissues. The suprachoroidal space can be expanded by fluid buildup because of some disease state in the eye or because of some trauma or surgical intervention. In some embodiments, the fluid buildup is intentionally created by the delivery, injection and/or infusion of a drug formulation into the suprachoroid to create and/or expand further the suprachoroidal space 36 (i.e., by disposing a drug formulation therein). This volume may serve as a pathway for uveoscleral outflow (i.e., a natural process of the eye moving fluid from one region of the eye to the other through) and may become a space in instances of choroidal detachment from the sclera.

To load the medicament into the ocular delivery device (also referred to herein as “delivery device”) for subsequent injection, in some embodiments, a system for delivering medicaments can include an extraction system for extracting medicaments or any fluid from a container, for example, a vial.

Although in this embodiment the aperture 22293 extends the length of the puncturing member 22291 that is disposed beyond the seal 22297 of the cap 22296, in some embodiments, the aperture 22293 can have an alternative arrangement, but still be configured to minimize or eliminate dead volume. In some embodiments, for example, a puncturing member could include multiple discrete and/or discontinuous apertures of the same or varying size, so long as at least one aperture is located at a low point of the collection area between the seal 22297 and the puncturing member 22291 such that any medicament at that low point (during extraction) can access the fluidic channel 22292, and therefore be accessible to the delivery device.
In some implementations, the length of a suitable aperture 22293 can be a function of the material properties of the puncturing member 22291. For example, if the puncturing member 22291 has a relatively low tensile strength, the puncturing member 22291 can become structurally unsound and easily broken if the aperture 22293 is relatively too large or too long. By using a more structurally rigid material for the puncturing member 22291, the aperture 22293 can be lengthened without significant fracture risk. In some implementations, for example, the puncturing member 22291 can be formed of metal, such as stainless steel, in part to provide sufficient structure stability while allowing for a sufficiently sized and placed aperture to avoid dead volume. In some implementations, the puncturing member can be formed of hard plastic, such as a polycarbonate. In some implementations, the puncturing member 22291 can be stiffer (e.g., substantially stiffer) than the seal 22297. In this manner, for example, minimal dead volume can be obtained.
In some implementations, for example, the aperture 22293 can extend to the bottom of the extraction member 22280 or further. Although not shown, the tethered sealing member 23400 is configured to be transitioned to an engaged configuration in which the tethered sealing member 23400 seals the fluidic channel 23286 of the engagement portion 23284. In this manner, in use, with the extraction member 23280 coupled to the medicament container (not shown), the tethered sealing member 23400 can be positioned in its engaged configuration to limit exposure of the medicament in the medicament container (not shown) to a volume external to the medicament container and extraction member 23280. When ready to extract the medicament into the delivery device, the tethered sealing member 23400 can be transitioned to its disengaged configuration to expose the fluidic channel 23286 and the engaement portion 23284, to allow for subsequent coupling to and extraction by the delivery device.
As described above, in use, a user can releasably couple the extraction member 23280 to the delivery device to establish fluid communication between the medicament containment chamber of the delivery device and the fluidic channel of the puncture member of the extraction member 23280. Then, the user can apply a force to urge the extraction member 23280 towards the container (not shown), and/or the container towards the extraction member 23280, such that extraction member 23280 is releasably coupled to the cap of the container. Furthermore, the puncturing member of the extraction member 23280 punctures the seal of the cap included in the container to establish fluid communication between the container and the medicament containment chamber via the extraction member 23280. Medicament can now be extracted from the container 21294 by a user by engaging the actuating member (not shown), as described herein with reference to system 19000 in the '139 Patent. After extraction, the user can decouple the extraction member 23280 from the delivery device, and transition the sealing member 23400 to its engaged configuration to seal the medicament within the medicament container. If the user desires another extraction of medicament, the user can disengage the sealing member 23480 and recouple the delivery device to the extraction member 23280 to access the medicament, as described above.
In this embodiment, as shown, the tethered sealing member 23400 is configured to surround the engagement portion 23284. In some embodiments, a sealing member can take other forms and/or shapes, and still be configured to seal the fluidic channel 23286. In some embodiments, for example, a sealing member can be configured to fit within the fluidic channel via an interference fit, e.g., to plug the fluidic channel and prevent exposure to the medicament, as described above.

The embodiments described herein can be formed or constructed of one or more biocompatible materials. Examples of suitable biocompatible materials include metals, glasses, ceramics, or polymers. Examples of suitable metals include pharmaceutical grade stainless steel, gold, titanium, nickel, iron, platinum, tin, chromium, copper, and alloys thereof. The polymer may be biodegradable or non-biodegradable. Examples of suitable biodegradable polymers include polylactides, polyglycolides, polylactide-co-glycolides (PLGA), polyanhydrides, polyorthoesters, polyetheresters, polycaprolactones, polyesteramides, poly(butyric acid), poly(valeric acid), polyurethanes and copolymers and blends thereof. A wide range of ocular diseases and disorders may be treated by the methods and devices described herein. Non-limiting examples of ocular diseases include uveitis, glaucoma, diabetic macular edema or retinopathy, macular degeneration, retinoblastoma, and genetic diseases. The methods described herein are particularly useful for the local delivery of drugs that need to be administered to the posterior region of the eye, for example the retinochoroidal tissue, macula, and optic nerve in the posterior segment of the eye. In one embodiment, the delivery methods and devices described herein may be used in gene-based therapy applications. For example, the methods may administer a fluid drug formulation into the suprachoroidal space to deliver select DNA, RNA, or oligonucleotides to targeted ocular tissues.
The microneedles can be used to target delivery to specific tissues or regions within the eye or in neighboring tissue. In various embodiments, the methods may be designed for drug delivery specifically to the sclera, the choroid, the Bruch's membrane, the retinal pigment epithelium, the subretinal space, the retina, the macula, the optic disk, the optic nerve, the ciliary body, the trabecular meshwork, the aqueous humor, the vitreous humor, and other ocular tissue or neighboring tissue in need of treatment.
A wide range of drugs may be formulated for delivery to ocular tissues using the present systems and devices described herein. Moreover, any of the delivery devices and/or methods described herein can involve, include and/or contain any of the drugs described herein. For example, in some embodiments, the medicament containment chamber 1310, 2310, 3310, or any other medicament containment chamber can contain any of the drugs and/or formulations described herein. As used herein, the term “drug” refers to any prophylactic, therapeutic, or diagnostic agent (e.g., a contrast agent). The drug may be selected from suitable proteins, peptides and fragments thereof, which can be naturally occurring, synthesized or recombinantly produced. Representative examples of types of drugs for delivery to ocular tissues include antibodies, anti-viral agents, chemotherapeutic agents (e.g., topoisomerase inhibitors), analgesics, anesthetics, aptamers, antihistamines, anti-inflammatory agents, and anti-neoplastic agents. In one embodiment, the drug is triamcinolone or triamcinolone acetonide.
The term “antibody” is intended to refer broadly to any immunologic binding agent such as IgG, IgM, IgA, IgD and IgE. An antibody can be monoclonal or polyclonal, and in one embodiment, is a humanized antibody. In one embodiment, a VEGF modulator is delivered via one of the devices described herein. In one embodiment, the VEGF modulator is a VEGF antagonist. In one embodiment, the VEGF modulator is a VEGF-receptor kinase antagonist, an anti-VEGF antibody or fragment thereof, an anti-VEGF receptor antibody, an anti-VEGF aptamer, a small molecule VEGF antagonist, a thiazolidinedione, a quinoline or a designed ankyrin repeat protein (DARPin). In one embodiment, an immunosuppressive agent is delivered via one of the devices described herein. In a further embodiment, the immunosuppressive agent is a glucocorticoid, cytokine inhibitor, cytostatic, alkylating agent, anti-metabolite, folic acid analogue, cytotoxic antibiotic, interferon, opioid, T-cell receptor directed antibody or an IL-2 receptor directed antibody. In one embodiment, the immunosuppressive agent is an anti-metabolite and the anti-metabolite is a purine analog, pyrimidine analogue, folic acid analogue or a protein synthesis inhibitor. In another embodiment, the immunosuppressive agent is an interleukin-2 inhibitor (e.g., basiliximab or daclizumab). Other immunosuppressive agents amenable for use with the methods and formulations described herein include, but are not limited to cyclophosphamide, nitrosourea, methotrexate, azathioprine, mercaptopurine, fluorouracil, dactinomycin, anthracycline, mitomycin C, bleomycin, mithramycin, muromonab-CD3, cyclosporine, tacrolimus, sirolimus or mycophenolate. In one embodiment, the drug formulation comprises an effective amount mycophenolate.
In one embodiment, the drug formulation delivered via one of the devices described herein comprises an effective amount of vascular permeability inhibitor. In one embodiment, the vascular permeability inhibitor is a vascular endothelial growth factor (VEGF) antagonist or an angiotensin converting enzyme (ACE) inhibitor. In a further embodiment, the vascular permeability inhibitor is an angiotensin converting enzyme (ACE) inhibitor and the ACE inhibitor is captopril.
In one embodiment, the drug is a steroid or a non-steroid anti-inflammatory drug (NSAID). In another embodiment, the anti-inflammatory drug is an antibody or fragment thereof, an anti-inflammatory peptide(s) or an anti-inflammatory aptamer(s).

Specific classes of NSAIDs that can be administered via the methods provided herein include salicylates, propionic acid derivatives, acetic acid derivatives, enolic acid derivatives, fenamic acid derivatives and cyclooxygenase-2 (COX-2) inhibitors. In one embodiment, the drug is a drug that inhibits, reduces or modulates the signaling and/or activity of PDGF-receptors (PDGFR). For example, in one embodiment, the PDGFR modulator is an anti-PDGF aptamer, an anti-PDGF antibody or fragment thereof, an anti-PDGFR antibody or fragment thereof, or a small molecule antagonist. In one embodiment, the PDGF antagonist is an antagonist of the PDGFRα or PDGFRβ. While the embodiments and methods herein describe delivering a medicament to a target tissue, the embodiments described herein can be configured to facilitate a biopsy procedure and/or removal of a substance from a target location.
While the embodiments have been described above in use on ocular tissue, in some instances, the embodiments and methods described herein can be used on any other suitable bodily tissue. For example, in some instances, the use of an adjustable length needle can be beneficial in conjunction with standard phlebotomy techniques during drug infusion and/or blood draw from a vein. Thus, while the embodiments and methods are specifically described above in use on ocular tissue, it should be understood that the embodiments and methods have been presented by way of example only, and not limitation.
While various embodiments have been described above, it should be understood that they have been presented by way of example only, and not limitation. Although the systems and methods are shown and described herein as providing for delivery of medicaments in the suprachoroidal space, in other embodiments, the systems and the methods described herein can be applicable for delivery of any suitable therapeutic substance to any portion of the eye, such as, the cornea, the retinal area or the vitreous. In other embodiments, any of the systems, methods and devices described herein can be used to deliver any suitable therapeutic substance to any desired target tissue (including non-ocular tissue).
