Claims
- 1. A percutaneous gastrostomy feeding tube apparatus for placement in a patient using an endoscope comprising:
a gastrostomy feeding tube having a first end, a second end; and a coupling member affixed to the gastrostomy feeding tube, the coupling member being configured such that it is accessible about the second end of the gastrostomy feeding tube such that the endoscope, or an instrument residing thereinside, can releasably couple with the coupling member so that the gastrostomy feeding tube and the endoscope can be introduced together through the alimentary tract and into the stomach of a patient and disengaged therewithin.
- 2. The apparatus of claim 1, wherein the coupling member comprises an elongate tract extending beyond the second end of the gastrostomy feeding tube and is sized such that the elongate tract can extend throughout a longitudinal channel of the endoscope.
- 3. The apparatus of claim 2, wherein the elongate tract comprises a thread or suture.
- 4. The apparatus of claim 3, wherein the elongate tract is of a braided configuration.
- 5. The apparatus of claim 3, wherein the elongate tract comprises a material having high tensile strength such that the elongate tract is resistant to breakage.
- 6. The apparatus of claim 2, wherein the elongate tract comprises a metallic wire.
- 7. The apparatus of claim 2, wherein the elongate tract is adapted to be drawn through the channel of the endoscope by an ancillary device disposed therein.
- 8. The apparatus of claim 2, wherein the gastrostomy feeding tube comprises a first portion that includes the first end, and a second portion that includes the second end, the first portion being adapted to be removed from the second portion such that the second portion can remain partially within in the patient to maintain external access to the stomach of the patient, and wherein the elongate tract is attached about the first portion.
- 9. The apparatus of claim 2, wherein the gastrostomy feeding tube further includes a distal loop with the elongate tract being attached about the proximal end thereof.
- 10. The apparatus of claim 1, wherein the coupling member comprises an integrated attachment region located about the second end, the integrated attachment region adapted to be coupled to an engaging member located about at least one of the endoscope or an ancillary device used therewith.
- 11. The apparatus of claim 10, wherein the coupling member comprises a hooked member.
- 12. A percutaneous gastrostomy feeding tube apparatus for placement using an endoscope comprising:
a gastrostomy feeding tube having a passageway extending therethrough, a first end, a second end, a first portion and a second portion, the first portion, which includes the first end, being configured and adapted for introducing the second portion within the stomach of a patient, whereby the first portion is subsequent removal from the second portion, the second portion including a passageway extending therethough and an end cap located about the second end, the end cap having an inner surface; and an elongate tract sized for passage through a channel of the endoscope, the elongate tract having a first end and a second end, the first end being attached about the first portion of the gastrostomy feeding tube, the tract extending through the passageway and beyond the end cap, wherein the distance of tract from the inner surface of the end cap to the second end of the tract exceeds the length of the endoscope channel.
- 13. The apparatus of claim 12, wherein the elongate tract extends at least 130 cm from the distal end of the gastrostomy feeding tube.
- 14. The apparatus of claim 12 wherein the elongate tract comprises a braided polymer.
- 15. In a percutaneous gastrostomy feeding tube for placement within the stomach of patient using an endoscope, the gastrostomy feeding tube having a passageway extending therethrough, a first portion which includes the first end, a second portion which includes the second end and which further includes an end cap having an inner surface, the improvement comprising:
a coupling member adapted to permit the gastrostomy feeding tube to be releasably coupled with the endoscope, the coupling member accessible about the second end of the gastrostomy feeding tube such that an engaging member located about at least one of the endoscope or an ancillary device deployed therewith can attach to the coupling member, thereby allowing the endoscope to be pulled through the alimentary tract of the patient along with the gastrostomy feeding tube during the placement thereof.
- 16. The combination in claim 15, wherein the coupling member comprises an elongate tract whose length exceeds that of a channel of the endoscope.
- 17. The combination in claim 15, wherein the coupling member comprises an integrated attachment region located about the second end, the integrated attachment region adapted to be coupled an engaging member located about at least one of the endoscope or an ancillary device used therewith.
- 18. A method of using an endoscope to verify correct placement of a percutaneous gastrostomy feeding tube apparatus within the stomach of a patient, wherein the percutaneous gastrostomy apparatus includes a coupling member that is attachable by an engaging member of at least one of the endoscope or an ancillary device used therewith, the method comprising the steps of:
a. coupling the gastrostomy feeding tube to an insertion wire that has been percutaneously introduced through a stoma into the stomach with the distal end thereof exiting the patient's mouth; b. attaching the endoscope to the coupling member using the engaging member; c. pulling the gastrostomy feeding tube and endoscope into the alimentary tract by withdrawing the insertion wire via the stoma; d. positioning the gastrostomy feeding tube with the stomach of the patient; and e. partially withdrawing the endoscope to verify correct placement of the gastrostomy feeding tube.
- 19. The method of claim 18, wherein the coupling member comprises an elongate tract, the method further including the step of pulling the elongate tract through the a internal channel of the endoscope prior to introducing the gastrostomy feeding tube and attached endoscope through the alimentary tract.
- 20. A percutaneous gastrostomy feeding tube apparatus for placement in a patient using an endoscope comprising:
a gastrostomy feeding tube having a passageway, a first portion, a second portion, a first end, a second end, and an end cap located about the second end, the first portion including a discardable dilator tip and distal loop; and an elongate tract comprising a polymer thread so dimensioned to reside within an internal channel of an endoscope, the elongate tract being affixed to the distal loop of the gastrostomy feeding tube and extending proximally through the passageway therefrom; wherein the elongate tract extends beyond second the end of the gastrostomy feeding tube by a sufficient length such that an ancillary device extending from a distal end of the endoscope, can capture and pull the elongate tract through the length of the internal channel, such that the endoscope can advance over the elongate tract to couple with the the gastrostomy feeding tube, thereby allowing both to be introduced together through the alimentary tract and into the stomach of a patient.
