Claims
- 1. An ultrasonic surgical instrument for emulsifying tissue from an operating site and removing the tissue by an aspiration flow force from a source comprising:
- a needle for receiving the ultrasonic energy, said needle having an internal aspiration passage terminating in a cavity with an inlet port to said cavity at the needle distal end, aspiration flow force applied to said aspiration passage aspirating tissue into said cavity through said inlet port to be emulsified by the ultrasonic energy while in said cavity; and
- an ultrasonic energy directing structure on the interior of said aspiration flow passage, said structure positioned in said aspiration passage proximally of said inlet port to modify the aspiration flow force pattern in said aspiration passage and having a first portion at an angle of less than 90.degree. relative to the longitudinal axis of said aspiration passage and facing toward said cavity for directing ultrasonic energy both distally of and non-parallel to the longitudinal axis of said aspiration passage to an area in said cavity to emulsify the tissue aspirated into said cavity, and a second portion proximal of said first portion at an angle to said aspiration passage and facing toward the proximal end of said needle for directing ultrasonic energy away from said inlet port along said aspiration passage back toward the proximal end of said needle to increase the effect of the aspiration flow in said aspiration passage, wherein at least the second portion is positioned on only one side of the aspiration passage so that the aspiration passage is asymmetrical in the second portion region.
- 2. An ultrasonic surgical instrument as in claim 1 wherein said first and second portions of said directing structure comprise at least one of a barrier, ramp, step, angled surface, bar, baffle, wedge and extension element disposed on an internal surface of said aspiration passage.
- 3. An ultrasonic surgical instrument as in claim 2 wherein said directing structure is one of an annular ramp or annular angled surface within said aspiration passage proximal of said cavity and said first portion is a surface of said ramp or angled surface angled downwardly toward the needle distal end to direct ultrasonic energy into said cavity and said second portion is a surface of said ramp or angled surface generally transverse to said aspiration passage to direct ultrasonic energy proximally generally along the axis of said aspiration passage.
- 4. An ultrasonic surgical instrument as in claim 3 wherein said ramp has a third surface portion between said first and second portions which is generally parallel to said aspiration passage.
- 5. An ultrasonic surgical instrument as in claim 2 wherein said ramp and said angled surface are annular and said directing structure first portion comprises the part of said ramp and angled surface that tapers downwardly toward said needle distal end.
- 6. An ultrasonic surgical instrument as in claim 5 wherein the distal end of said needle defining said cavity distally of said directing structure surface converges to said inlet port with the converging cavity directing the ultrasonic energy toward the cavity center.
- 7. An ultrasonic surgical instrument for emulsifying tissue from an operating site and removing the tissue by an aspiration flow force from a source comprising:
- a needle for receiving the ultrasonic energy, said needle having an internal aspiration passage terminating in a cavity with an inlet port to said cavity at the needle distal end, aspiration flow force applied to said aspiration passage aspirating tissue into said cavity through said inlet port to be emulsified by the ultrasonic energy while in said cavity; and
- an ultrasonic energy directing structure on the interior of said aspiration flow passage, said structure positioned in said aspiration passage proximally of said inlet port to modify the aspiration flow force pattern in said aspiration passage and having a first portion at an angle of less than 90.degree. relative to the longitudinal axis of said aspiration passage and facing toward said cavity for directing ultrasonic energy both distally of and non-parallel to the longitudinal axis of said aspiration passage to an area in said cavity to emulsify the tissue aspirated into said cavity, and a second portion at an angle to said aspiration passage and facing toward the proximal end of said needle for directing ultrasonic energy away from said inlet port along said aspiration passage back toward the proximal end of said needle to increase the effect of the aspiration flow in said aspiration passage, wherein said cavity has a longitudinal axis and said needle has a transition portion along said aspiration passage to offset said cavity longitudinal axis spaced from and generally parallel to said needle longitudinal axis and said directing structure first portion comprises said transition portion.
- 8. An ultrasonic surgical instrument as in claim 7 wherein said transition portion has a second surface portion generally transverse to said aspiration passage to direct ultrasonic energy back into said aspiration passage in a proximal direction.
- 9. An ultrasonic surgical instrument for emulsifying tissue from an operating site and removing the tissue by an aspiration flow force from a source comprising:
- a needle for receiving the ultrasonic energy, said needle having an internal aspiration flow passage terminating in a cavity with an inlet port at the needle distal end and said inlet port being positioned in the needle with an entrance to said cavity and at least a part of said cavity being at an angle to the longitudinal axis of said aspiration passage and the distal end of said aspiration passage longitudinal axis aligned directly into said cavity, aspiration flow force applied to said aspiration passage aspirating the tissue into said cavity through said inlet port to be emulsified by the ultrasonic energy in said cavity; and
- a directing structure in said cavity both proximally and distally of said inlet port, said directing structure having surfaces for directing the ultrasonic energy in said cavity toward the entrance of said cavity from said inlet port to emulsify the tissue aspirated into said cavity; and
- wherein the needle distal end is pointed with an apex aligned with the internal passageway proximal of said cavity.
- 10. An ultrasonic surgical instrument as in claim 9 wherein said directing structure includes a first portion which comprises opposing surfaces of at least a part of said cavity which direct the ultrasonic energy into the interior of said cavity.
- 11. An ultrasonic surgical instrument as in claim 10 wherein said directing structure also includes a second portion which comprises a flat surface at the distal end of said cavity generally transverse to said aspiration passage to direct ultrasonic energy proximally in said aspiration passage.
- 12. An ultrasonic surgical instrument as in claim 9 wherein said directing structure has a surface portion for directing ultrasonic energy away from said cavity into said aspiration passage back toward the proximal end of said needle to increase the effect of the aspiration flow in said aspiration passage.
- 13. An ultrasonic surgical instrument as in claim 9 wherein the pointed distal end is one of sharp or blunted.
- 14. An ultrasonic surgical instrument for emulsifying tissue from an operating site and removing the tissue by an aspiration flow force from a source comprising:
- a needle for receiving the ultrasonic energy, said needle having an internal aspiration flow passage terminating in a cavity with an inlet port at the needle distal end, aspiration flow force applied to said aspiration passage aspirating the tissue into said cavity through said inlet port to be emulsified by the ultrasonic energy in said cavity; and
- said distal end of said needle having an enlarged outer perimeter relative to the remainder of said needle and said inlet port and said cavity are both in said enlarged end and at an angle to the needle longitudinal axis, the longitudinal axis of said aspiration passage and the distal end of said aspiration passage longitudinal axis aligned directly into said cavity, the internal wall of said enlarged end surrounding said inlet port and forming a first surface for focusing ultrasonic energy across said inlet port to emulsify tissue aspirated into said cavity; and
- wherein the needle distal end is pointed with an apex aligned with the internal passageway proximal of said cavity.
- 15. An ultrasonic surgical instrument as in claim 14 wherein the internal wall of said enlarged distal end has a depression forming a second surface to direct ultrasonic energy back into said aspiration passage in a proximal direction.
- 16. An ultrasonic surgical instrument as in claim 14 wherein the internal wall of said enlarged distal end has a wall adjacent said inlet port substantially transverse to said aspiration passage forming a second surface to direct ultrasonic energy back into said aspiration passage in a proximal direction.
- 17. An ultrasonic surgical instrument as in claim 14 wherein the internal wall of said enlarged distal end that is angled relative to said needle also forms a second surface to direct ultrasonic energy proximally in said aspiration passage.
- 18. An ultrasonic surgical instrument for emulsifying tissue from an operating site and removing the tissue by an aspiration flow force from a source comprising:
- a needle for receiving the ultrasonic energy, said needle having an internal aspiration passage terminating in a cavity with an inlet port to said cavity at the needle distal end, aspiration flow force applied to said aspiration passage aspirating tissue into said cavity through said inlet port to be emulsified by the ultrasonic energy while in said cavity; and
- an ultrasonic energy directing structure on the interior of said aspiration flow passage, said structure comprising a baffle extending across said aspiration passage proximally of said inlet port to modify the aspiration flow force pattern in said aspiration passage and having a first portion at an angle of less than 90.degree. relative to the longitudinal axis of said aspiration passage and facing toward said cavity for directing ultrasonic energy both distally of and non-parallel to the longitudinal axis of said aspiration passage to an area in said cavity to emulsify the tissue aspirated into said cavity, and a second portion at an angle to said aspiration passage and facing toward the proximal end of said needle for directing ultrasonic energy away from said inlet port along said aspiration passage back toward the proximal end of said needle to increase the effect of the aspiration flow in said aspiration passage.
- 19. An ultrasonic surgical instrument as in claim 14 wherein the pointed distal end is one of sharp or blunted.
