Claims
- 1. A prescription sheet in combination with a medical record, the combination comprising:a prescription sheet having a first and a second side with an area for writing a prescription which is lined with lines being a pre-selected distance apart formed on the first side of the prescription sheet wherein the prescription sheet further includes reproduction material positioned on the second side of the prescription sheet at a location corresponding to the area for writing the prescription wherein the reproduction material is exposed such that information written in the area for writing prescriptions can be transferred to a medical record that is unattached to the prescription sheet by placing the prescription sheet over the medical record and writing in the area for writing the prescription; and a medical record that includes a patient history area for writing medical records for the patient wherein the patient history area is lined with the lines being the pre-selected distance apart corresponding to the line separation of the prescription sheet such that a prescription writer can position the prescription sheet on the medical record so that the reproduction material is in contact with the patient history area of the medical record and align the lines of the prescription sheet with lines of the patient history area so that when the prescription writer writes the prescription, the reproduction material reproduces the prescription directly onto the medical record, wherein the prescription is transferred onto the medical record without requiring any surface treatment of the medical record.
- 2. The combination of claim 1, wherein the reproduction material of the prescription sheet comprises a carbonized material.
- 3. The combination of claim 1, wherein the reproduction material is positioned on the second side of the prescription sheet in manner such that a first distance separates at least one edge of the reproduction material from at least one edge of the prescription sheet.
- 4. The combination of claim 3, wherein the first distance is approximately 0.5 inch.
- 5. The combination of claim 1, wherein the pre-selected distance separating the lines on the first side of the prescription sheet is about {fraction (5/16)} inch.
- 6. The combination of claim 1, wherein the first side of the prescription sheet comprises a first area for the prescription writer to record the name of the drug being prescribed, a second area for the prescription writer to record the drug dosage, and a third area for the prescription writer to record refill information.
- 7. The combination of claim 1, wherein the prescription writer is a physician.
- 8. The combination of claim 1, wherein the prescription writer is a dentist.
- 9. The combination of claim 1, wherein the prescription comprises over-the-counter medication.
- 10. A method of simultaneously writing prescription and recording the prescription onto a medical record comprising:providing a prescription sheet that has a prescription writing area with lines that are selected to be a first distance apart on a front sheet and has exposed reproduction material on a second side opposite the prescription writing area; placing the prescription sheet onto the medical record such that the lines of a prescription area of the prescription sheet are substantially aligned with the lines of the medical record and such that the reproduction material is placed in contact with the prescription area of the prescription sheet; and writing the prescription in the prescription area while substantially maintaining the alignment between the prescription sheet, wherein writing the prescription area results in a reproduction material of the prescription sheet transferring the written prescription directly onto the medical record, wherein the written prescription can be transferred to the prescription area without requiring any surface treatment of the prescription area.
- 11. The method of claim 10, wherein placing the prescription sheet onto the medical record further comprises placing the prescription sheet adjacent to an area containing examination and diagnosis information that the prescription is intended to treat.
- 12. The method of claim 10, wherein placing the prescription sheet onto the medical record further comprises placing the prescription sheet adjacent to an area on the medical record containing the date which the prescription is given.
- 13. The method of claim 10, wherein writing the prescription comprising writing information relating to an over-the-counter medication.
- 14. A prescription paper for a prescription writer to transfer prescription information directly onto a patient's record having a plurality of adjacent lines spaced to receive information, comprising:a front side having a selected area for the prescription writer to write down prescription information for patients, wherein the selected area comprises at least two adjacent lines separated by a pre-determined distance, wherein the pre-determined distance is substantially equal to the distance between two adjacent lines located on the patient's record; a back side having an exposed reproduction material, wherein the reproduction material is exposed so as to be positioned directly on the patient's record in a manner such that the at least two adjacent lines on the selected area of the prescription paper aligns with the adjacent lines on the patient's record so that the reproduction material transfers the prescription information directly onto the space between the adjacent lines on the patient's record while the prescription writer is writing down the prescription information, wherein the reproduction material transfers the prescription information onto the patient's record without requiring any surface treatment of the patient's record.
- 15. The prescription paper of claim 14, wherein the reproduction material comprises a carbonized material.
- 16. The prescription paper of claim 14, wherein the reproduction material occupies less than about 70% of the area of the back side of the prescription paper.
- 17. The prescription paper of claim 14, wherein the selected area comprises at least seven lines.
- 18. The prescription paper of claim 14, wherein the pre-determined distance is approximately {fraction (5/16)} inch.
